阿德福韦酯对HBeAg阳性慢性乙型肝炎患者外周血T细胞亚群的影响

摘　　要：目的探讨阿德福韦酯(ADV-dpv)抗病毒治疗对HBeAg阳性慢性乙型肝炎患者外周血T细胞亚群的影响。方法选择57例HBeAg阳性慢性乙型肝炎患者,均给予ADV-dpv 10 mg,每日1次,共48周,采用流式细胞术检测治疗前、治疗3个月和6个月患者外周血T细胞亚群。结果在抗病毒治疗前,与低病毒载量患者相比,高病毒载量患者存在着更为明显的T细胞亚群的失衡,差异有统计学意义(P<0.05);随着抗病毒治疗的进行,T细胞亚群失衡呈进行性改善,除CD3+T淋巴细胞较治疗前明显提高,差异有统计学意义(P<0.05)外,其余指标与治疗前比较,差异均无统计学意义(P>0.05);ADV-dpv治疗后HBV-DNA转阴组与未转阴组患者初始T细胞亚群的差异均无统计学意义(P>0.05)。结论HBeAg阳性慢性乙型肝炎患者的病毒载量与其外周血T细胞亚群的状态密切相关,经ADV-dpv抗病毒治疗后其外周血T细胞亚群的状态呈逐渐改善趋势。Objective To observe the effect of adefovir dipivoxil on T lymphocyte subsetes the treatment of HBeAg-positive chronic hepatitis B.Methods Totally 57 patients with HBeAg-positive chronic hepatitis B were treated with adefovir dipivoxil 10 mg/d for 48 weeks.The T lymphocyte subset in peripheral blood was analyzed by FCM before and after treatment for 3 and 6 months.Results The inferior T lymphocyte subsetes in peripheral blood correlated with the high level of pretreatment HBV-DNA（〉107copies/ml）（P〈0.05）.CD3＋,CD4＋,the ratios of CD4＋/CD8＋ rose and CD8＋ descended after treated with adfovir dipivoxil for 24 weeks,which had no statistic difference（P〉0.05） but CD3＋（P〈0.05）.The com-parison of T lymphocyte subsetes in peripheral blood before treatment between the patients who had a negative conversion of HBV-DNA and the patients who hadn’t there was no statistic difference（P〉0.05）.Conclusion The level of HBV-DNA correlated with the abnomality of T lymphocyte subsetes in pe-ripheral blood in patients with HBeAg-positive chronic hepatitis B.Adfovir dipivoxil could improve the abnormality of T lymphocyte subset in peripheral blood in patients with HBeAg-positive chronic hepati-tis B in a certain extent.
