微创经皮肾穿刺钬激光碎石术15例围术期护理

摘　　要：目的:探讨微创经皮肾穿刺钬激光碎石术治疗肾结石患者围术期的护理方法。方法:对15例肾结石患者行微创经皮肾穿刺钬激光碎石术,术前给予人文关怀、心理疏导、术前准备,术后严密观察生命体征变化,正确指导活动与饮食,加强肾穿刺造瘘管、双J管、导尿管护理,注意防治感染,重视舒适护理、健康指导。结果:本组11例行一期穿刺取石,4例行二期取石,无中途改开放手术病例;出院前复查X线,肾盂结石及梗阻全部解除12例,肾盏少量结石残留3例,残留肾结石行体外冲击波碎石治疗;随访效果均满意,平均住院9 d。结论:微创经皮肾穿刺钬激光碎石术具有微创、并发症少、结石清除率高、恢复快、住院时间短的优点,围术期给予精心护理,可减少并发症的发生,提高患者满意度,值得临床推广应用。Objective： To study the perioperative nursing methods in the treatment of kidney stone by minimally invasive percutaneous nephrolithotomy with holmium laser. Methods： The 15 patients were given humane care, psychological induction, before operation and close postoperative observation on the changes in vital signs, the correct guidance on activities and diet was conducted, strengthening the care for the puncture - made kidney fistula, double - J tube and catheter; paying attention to preventing from infections, and to comfort care and health instruction. Results： The first phase percutaneous nephrolithotomy was applied to 11 patients and the second phase percutaneous nephrolithotomy to 4 patients , and the operation was not used instead. According to the X -ray exam, 12 patients released from pyelolithiasis and obstruction; 3 patients had a small amount of residual calyceal calculi and were treated with extracorporeal shock wave lithotripsy treatment. The effectiveness was satisfactory, the av- erage length of stay in hospital was 9 days. Conclusion： The minimally invasive percutaneous nephrolithotomy holmium laser lith- otripsy has the advantages of minimal wound, fewer complications, high clearance rate, quick recovery and shorter hospitalization. Perioperative nursing care can reduce complications jimprove patient satisfaction and it is worthy of application in clinic.
