我院81例洋葱伯克霍尔德菌感染患者的抗菌治疗分析

摘　　要：目的分析洋葱伯克霍尔德菌(BC)感染患者的临床特征及分离菌的体外药物敏感性,探讨该菌所致感染的治疗特点。方法回顾性分析2005年1月至2007年12月我院BC感染81例患者的感染类型、临床表现及易感因素等,分析分离菌对抗菌药的敏感性。结果81例均属医院感染,包括肺部感染62例、血流感染11例、尿路感染5例及伤口感染3例。所有患者均有神经系统疾病、恶性肿瘤等基础疾病及使用呼吸机等易感因素。对于该菌敏感率超过80%的抗菌药有头孢他啶、美罗培南、哌拉西林/他唑巴坦;较敏感的药物尚有复方磺胺甲口恶唑、环丙沙星、头孢哌酮/舒巴坦。结论BC感染都为院内感染,感染患者都有基础疾病或易感因素;该菌对抗菌药物的耐药性高,应根据药敏结果选用抗菌药。AIM To analyze the clinical characteristics of Burkholderia cepacia（B, cepacia） infections and in vitro antimicrobial susceptibilities to clinical isolates of B. cepacia, and to discuss antimicrobial therapy for the infections caused by this bacterium. METHODS The clinical features including types of infections, clinical manifests and risk factors of 81 cases of B. cepacia infections from January 2005 to December 2007 were analyzed retrospectively. The antimicrobial susceptibilities of clinical strains were analyzed. RESULTS All 81 cases were belonged to hospital-acquired infections including 62 cases of pneumonia, 11 cases of blood-stream infections, 5 of urinary infections and 3 of wound infections. All cases had underlying diseases such as serious nerve system infections, malignant tumors or risk factors such as mechanical ventilation. Clinical isolates of B. ceptwia had the susceptibility rates over 80% only for ceftazidime, meropenem and piperacillin-sulbactam and were relatively susceptible sulfamethoxazole-trimethoprim, ciprofloxacin and cefoperazone- sulbactam. CONCLUSION All of the infections caused by B. cepacia were hospitalacquired infections and all of the patients had underlying diseases or risk factors. Clinical isolates of B. cepacia were highly resistant to antimicrobials, so antimicrobials should be chosen according to the susceptibility testing results for the treatment of B. cepacia infections.
