前列腺素E_2凝胶和催产素促宫颈成熟和引产效果的比较

摘　　要：比较产道内局部应用ＰＧＥ２凝胶与静脉滴注催产素促宫颈成熟及引产效果。将１００例单胎，头位，足月妊娠的初产妇随机分为两组，每组各５０例，一组于宫颈管内或阴道后穹隆局部应用ＰＧＥ２凝胶－ＰＧＥ２组，一组用静脉滴注催产素作为对照组。To compare the effects of prostaglandin E2 (PGE2 )gel used intracervically or in posterior fornix of vagina and oxytocin administered intravenously. One hundred full term primiparas of single vertex position were divided into two groups,50 in each group. The rates of successful cervical ripening and labour induction 12-hours and 32-hours after the gel administration in PGE2 group were 96% and 96% respectively. But in oxytocin group the rates were 30% and 56% respectively(x2 =46.72 and 22.46 respectively P< 0.005). The rate of spontaneous vaginal delivery was 84% (the oxytocin group: 66% , x2 = 3.32, P < 0.05). The mean value of total labour stage was 5.6±0.8h(the oxytocin group:8.4± 3.5h,P<0.05). The rate of cesarean section was 10% (the oxytocin group: 24% , P < 0.05). There were no differences between the two groups in postparturn haemorrhage and in weight of the newborn. The effect of PGE2 gel for cervical ripening is better than oxytocin. The pharmacological action of PGE2 gel is safe and reliable. The mechanism of PGE2 was discussed.
