摘 要:目的:建立一种改良萃取-高效液相色谱(HPLC)法测定全血中环孢菌素A(Csa)浓度.研究亲代活体肝移植患者全血中Csa浓度的早期变化及影响因素.方法:血样经两种复合溶剂分三步萃取后,以Kromasil C_(18)(150 mm×4.6mm,5μm)为分析柱,乙腈-甲醇-水-异丙醇(5.7:2.4;1.8:O.1)为流动相,71℃柱温下进行色谱分离.采用二氢奎尼(DQI))为内标,在 212nm处紫外定量检测.结果:全血中Csa最终最低检测浓度为26.0 μg/L,线性范围为52.μg/L~1248.0 μg/L.,r—0.9986,方法平均回收率为(99.12士4.31)%,日内RSI)≤4.6%(n=6),日间RSI)≤6.23%(n=6).结论;本法快速、灵敏、专一性强、内源性物质及其它各药组分不干扰测定.本法被用于亲代活体肝移植患者术后体内血药浓度的分析测定.结果表明:亲代活体肝移植患者术后早期全血中CSA浓度变化所受影响因素较复杂,To establish a high performance liquid chro-matographic method utilizing a improved extraction procedurefor the determination of cyclosporin A(CsA) in human whole blood and the study of early changing and affecting factors of CsA in the patient whole blood after parental living liver-related-transplantation. Methods: The drug was extracted from whole blood with modification and the analytical column is kromasil C18(150 mm×A. 6 mm, 5Um). the mobile plase consists of mixture of acetonitrile-methanol-water-isopropanol (5. 7:2. 4:1. 8 :0. 1 ) and the column temperature was maintained at 71℃. CsA is detected at 212 nm with dihydro-quinidine as internal standard. Results: The detection limit of CsA is 26. 0μg/L in whole blood. The method was linear within the range of 52. 0μg/L~1248. 0μg/L (r=0. 9986, n =6). The mean recovery from whole blood is (99. 12± 4. 31)%. The RSD of within-day and day-to-day are less than 4. 60% and G.23%, respectively. Conclusion: The method is simple, rapid and sensitive. There are no interference of the endogenous component or the other drugs after hepatic transplantation. In this study, the method is applied to measure CsA in patient whole blood after parental living liver-related-transplantation. The result indicate that a several of factors relative to early changing of CsA in patient whole blood.
