银屑病患者血液流变学与血脂及载脂蛋白的相关性研究

摘　　要：目的:探讨银屑病患者血液流变性变化与血脂及载脂蛋白水平的相关性,以及在银屑病发病中的作用。方法:对银屑病患者多项血液流变性指标以及血清总胆固醇(TC)、总甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-c)、低密度脂蛋白胆固醇(LDH-c)和载脂蛋白A1(apoAI)、载脂蛋白B100(apoB100)进行检测,并与正常人进行综合对比分析。结果:银屑病组与对照组比较:全血粘度(ηb)、血浆粘度(ηP)、红细胞聚集指数(EAI)、红细胞硬度(TK)、血沉(ESR)、血沉K值、血清TC、TG、LDL-c及apoB100均明显升高(P均<0.01),apoAI、HDL-c下降,红细胞压积(HCT)、纤维蛋白原无明显差异(P均>0.05)。银屑病组ηb与TC、LDL-c、TG、apoB100呈明显正相关;ηP与TC、TG呈明显正相关;EAI、ESR与TC、TG明显正相关;TK与TC明显正相关。结论:银屑病患者血液流变性、血脂及载脂蛋白水平明显异常;血脂升高使血浆粘度升高,同时造成红细胞膜成分改变导致红细胞变形能力下降,从而引起血液粘度升高造成微循环障碍和内皮细胞损伤,可能是银屑病发病的重要原因之一。Objective To explore the relationship between hemorrheologic property and blood lipid and apolipoprotein levels in patients with psoriasis, and investigate their roles in pathogenesis of psoriasis. Methods The indexes of hemorrheologic property and the serum levels of TC ,TG, HDL-c, apoAI, apoB100 in patients with psoriasis were detected and were compared with that in normal control. Results Compared with the control group, the ηb,ηP, EAI, TK, ESR, ESR K value, serum TC, TG, LDL-c and apoB100 in patientswere significantly increased in patients with psoriasis （P 〈 0.01 ）, and there was no significant different in apoAI and HDL-c, HCT, fihrinogen level（ P 〉 0.05, respectively）. ηb was positively correlated with TC, LDL-c, TG and apoB100 ;ηP was positively correlated with TC and TG, EAI and ESR was positively correlated with TC and TG, TK was positively correlated with TC in psoriasis group, respectively. Conclusion The hemorrheologic property and the levels of blood lipid and apolipoprotein are significant abnormal in patients with psoriasis ; the increased plasma viscosity is caused by the increase of blood lipid, simultaneously induce the change of erythroeyte membrane lipid composition and degrade the erythroeyte deformability, furthermore, microcirctdation disturbance and endothelial cells injury are induced, which may play an important role in pathogenesis of psoriasis.
