益气化瘀补肾方对大鼠血瘀型颈椎病作用机制研究

摘　　要：目的:探讨益气化瘀补肾方对血瘀型颈椎病大鼠的作用机制。方法:选择3月龄雌性SPF级SD大鼠30只,随机分为正常组、血瘀型颈椎病模型组(模型组)和益气化瘀补肾方组(治疗组),每组10只。正常组不施加任何手术,其余两组采用动静力失衡性大鼠颈椎间盘退变模型复合肾上腺皮质激素和肾上腺素法建立血瘀型颈椎病模型。手术3个月后,治疗组大鼠予益气化瘀补肾方灌胃治疗1个月。通过放射免疫学、颈椎间盘组织病理学、免疫组织化学、分子生物学等技术研究益气化瘀补肾方作用机制。结果:与正常组相比,模型组动物出现舌质瘀紫、尾色瘀青,血黏度、CD62p增高,cAMP增高,颈椎间盘退变,Ⅱ型胶原表达减少;治疗组与模型组比较,血浆黏度、CD62p、三酰甘油、cAMP降低(P<0.05或P<0.01);颈椎间盘退变有所改善,Miyamoto评分下降(P<0.05),Ⅱ型胶原表达增加(P<0.01),Agcl、Col2a1、TIMP-1表达增高(P<0.05)。结论:益气化瘀补肾方可通过免疫系统、代谢系统、凝血系统、内分泌系统等改善血瘀型颈椎病大鼠的证候指标,延缓椎间盘退变。Objective： To explore the mechanisms of ＂Yiqi Huayu Bushen Recipe＂（YHBR） , a compound traditional Chinese herbal medicine, in treating cervical spondylopathy （CS） of blood stasis pattern in rats. Methods： A total of 30 three-month-old female Sprague-Dawley rats were randomly divided into normal control, model group and treatment group ; there were ten rats in each group. The rat model of cervical spondylopathy of blood stasis pattern was established by combining disease and pattern models. After one-month YHBR treatment in treatment group, all rats were sacrificed. Serum, plasma and cervical intervertebral discs were detected and observed by radio-immunology, histopathology, immunohistochemistry, etc. Results： Com- pared with those in the normal control group, rats in the model group were found bruise tongue and blue tail, hemorheological parameters and the expression of CD62p were increased, the content of cAMP was increased, the intervertebral disc structure was destructed and type Ⅱ collagen protein expression decreased. Compared with the model group, rats in treatment group status recovered more quickly, the hemorheological parameters, the expression of CD62p and content of triglyceride and cAMP were decreased （P 〈 0.05, P 〈 0.01 ）, intervertebral disc structure was improved, Miyamoto score decreased （ P 〈 0.05 ）, type Ⅱ collagen protein expression in annulus fibrosus increased as the expressions of Agcl, Col2al and TIMP-1 mRNAs were higher in the treatment group. Conclusion： YHBR may alleviate symptoms of pattern and delay degeneration of cervical intervertebral disc by immune-metabolism system, metabolism system and coagulation system,hence to treat CS of blood stasis pattern.
