摘 要:目的探讨胃癌患者外周血单个核细胞(PBMC)HLA-A、HLA-B、HLA-DP、HLA-DR、HLA-G基因的表达水平及对胃癌的临床应用价值。方法设计HLA-A、HLA-B、HLA-DP、HLA-DR、HLA-G基因引物和实时荧光PCR相对定量法反应体系,在ABI7500实时荧光定量PCR仪上检测43例胃癌患者和22例健康人PBMC内基因表达水平并分析其差异。结果(1)胃癌患者HLA-A、HLA-B、HLA-DP、HLA-DR、HLA-G的mRNA相对表达量分别为0.71±0.26、0.61±0.37、0.91±0.33、0.90±0.38、0.77±0.27。健康组分别为1.23±0.60、1.17±0.60、1.07±0.68、1.12±0.55、0.98±0.37。与健康组相比,胃癌患者HLA-A、HLA-B、HLA-G表达水平降低,差异有统计学意义。HLA-DP和HLA-DR则无明显差异。(2)在不同分化的胃癌患者中,HLA-A、HLA-B、HLA-DP、HLA-DR和HLA-G的mRNA表达水平无明显差异。胃癌Ⅰ期+Ⅱ期的HLA-G表达水平高于Ⅲ期+Ⅳ期,差异有统计学意义。不同分期的HLA-A和HLA-B的表达水平无明显差异。结论胃癌患者PBMC的HLA-A、HLA-B、HLA-G的基因表达水平下调且HLA-G不同分期间存在差异。检测PBMC的HLA-A、HLA-B、HLA-G基因表达水平有助于胃癌的诊断和研究。Objective To investigate the expression levels of HLA-A,-B,-DP,-DR,-G gene in peripheral blood mononuclear cell (PB- MC) of gastric cancer patients and its clinical significance. Methods The mRNA of HLA-A, -B, -DP, -DR, -G in PBMC was detected by real-time relative quantitative PCR in 43 patients with gastric cancer and 22 healthy individuals. Results The relative expression levels of HLA-A, -B, -DP, -DR, -G in PBMC were 0.71 ±0.26, 0.61 ±0.37, 0.91 ±0.33, 0.90 ±0.38, 0.77 ±0.27 in gastric cancer group and 1.23 ±0.60, 1.17 ±0. 60, 1.07 ±0.68, 1.12 ±0.55, 0.98 ±0.37 in healthy control group, respectively. The expression levels of HLA-A, -B, -G in PBMC of gastric cancer group was significandy lower than those of control group, while there was no significantly difference for HLA-DP and -DR between the two groups. No correlation was found between the relative expression levels of HLA-A, -B and -G in PBMC and the clinical stage, histological grade of gastric cancer except that the expression level of HLA-G in Ⅲ + Ⅳ grade was lower than that in Ⅰ + Ⅱ grade of gastric cancer. Conclusions HLA-A, -B, -G may play critical role in immunity response of tumor. The expression levels of HLA-A, -B and -G in PBMC were down-regulated and significantly different from each other when the gastric cancer occurs. The detection of HLA genes expression should be helpful for the diagnosis and research on gastric cancer.
