摘  要:【目的】探讨微米大黄炭治疗胃溃疡出血的止血作用机制。【方法】选用昆明种小鼠和SD大鼠为研究对象,随机分为空白对照组、云南白药组(剂量为9.gkg-1.d-1)和微米大黄炭高、中、低剂量组(剂量分别为8、4、2.gkg-1.d-1),灌胃给药6 d后检测小鼠出、凝血时间及血小板计数;测定大鼠血小板功能及纤溶活性等指标。【结果】与空白对照组比较,微米大黄炭各剂量组均能缩短小鼠出、凝血时间(P<0.01),增加血小板计数(P<0.05或P<0.01),提高大鼠血小板聚集性,上调血栓烷素B2(TXB2)而下调6-酮-前列腺素F1α(6-keto-PGF1α)水平(P<0.05或P<0.01),升高血小板颗粒膜蛋白-140(GMP-140)含量(P<0.05或P<0.01);缩短大鼠凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)(P<0.01),而对组织型纤溶酶原激活剂(t-PA)和组织型纤溶酶原抑制剂(PAI-1)活性无显著影响(P>0.05)。【结论】通过激活内源性和外源性途径的多种凝血因子而促进凝血过程中的凝血酶原和凝血活酶的生成,增加血小板计数和聚集性可能是微米大黄炭止血的重要作用机制。Objective To explore the hemostatic mechanism of micron rhubarb charcoal(MRC) for the treatment of gastric ulcer bleeding.Methods Both Kunming mice and SD rats were used as the experimental animals,and were randomized into blank control group,Yunnan white powder(9g·kg-1·d-1)group,and high-,middle-and low-dose MRC(8,4 and 2g·kg-1·d-1)groups,respectively,ig for 6 days.After treatment,bleeding time(BT),coagulation time(CT),and platelet count in mice were detected,and the platelet function and fibrinolytic activity in rats were examined.Results In mice MRC groups,BT and CT were shortened(P〈0.01),and the number of platelet was increased(P〈0.05) as compared with the black control group.In rats MRC groups,platelet aggregation was increased,thromboxane(TXB2)content was up-regulated and 6-ketone prostaglandin(6-keto-PGF1a)content was down-regulated(P〈0.05 or P〈0.01),platelet granular membrane protein(GMP-140) content was increased(P〈0.05 or P〈0.01),and prothrombin time(PT) and activated partial thromboplastin time(APTT) were shortened(P〈0.01).However,MRC had no effect on rats plasminogen activator(t-PA) and its inhibitor(PAI-1)contents.Conclusion The hemostatic mechanism of MRC for the treatment of gastric ulcer bleeding is probably related to enhancing the production of prothrombin and thromboplastin,and increasing the number and aggregation of platelet through activating the endogenous and exogenous coagulation factors.
