摘　　要：目的了解河南省献血地区某行政村人类免疫缺陷病毒(HIV)的感染状况及流行因素。方法对户口属于该村的全体居民进行HIV入户调查,收集该村已经进行HIV检查的记录。结果全村总共352户家庭,总人口1529人,68.5%的居民共1047人参加了筛查;HIV阳性居民139例,总阳性率为13.3%;全村33.5%的家庭有HIV血清阳性成员;有偿献血员的阳性率为27.4%,HIV阳性居民中,有64.7%为献血感染,有0.7%为输血感染,2.2%为性接触感染,5.0%为母婴感染;97.1%的HIV感染者进行了CD4^+T淋巴细胞的检测,<200×10~6/L者18例。结论河南省献血地区某些村庄HIV感染率较高,过去主要以献血感染为主,性接触和母要传播已成为目前HIV感染的主要途径,应采取相应的措施控制其进一步传播。Objective To investigate HIV prevalence and epidemiological factors among the villagers in the commercial blood donating communities of Henan Province. Methods All residents of the village were followed to undergo an indoor interview to finish a questionnaire of HIV prevalence. We collected the records of the information about the village residents who had HIV serologic testing. Results There were total 1529 residents in 352 households in the village including 1 047 residents who participated in the HIV serologic screening test. A total of 139 HIV seropositive residents were found in the HIV screening. The percentage of HIV infection of the whole population was 13.3%. 33.5% households had at least one member infected with HIV. The percentage of HIV infection among the paid blood donors was 27.4%. The proportions of HIV infection through routes of blood tranfusion, sex and mother-to-child were respectively 0.7%, 2.2% and 5.0% among 139 HIV seropositive residents. 97.4% of the HIV seropositive residents were measured for CD4^＋ T lymphocyte. There were 18 HIV seropositive residents whose number of CD4^＋ T was less than 200 × 106/L. Conclusion Some villages in the commercial blood donating region of Henan Province show high HIV prevalence. The former paid blood donors should be responsible for the main transmission route in these villages. However, sexual transmission and mother-to-child transmission have become the main routes in general population. Effective action should be taken to control the spread of HIV.
