实施大面积孕产妇HIV抗体检测及预防HIV母婴传播服务效果评价

摘　　要：[目的]了解大面积开展孕产妇艾滋病病毒(HIV)抗体检测及对HIV抗体阳性孕产妇宣传落实预防HIV母婴传播工作开展情况,控制HIV母婴传播发生,并评价其效果。[方法]收集、统计河南省周口市8个HIV感染者300人以上县市,2003年4月份以来的预防HIV母婴传播工作报表、数据库信息,评价孕产妇HIV抗体检测完成情况、预防性干预措施落实及效果情况。[结果]8县市基本能按要求完成孕产妇HIV抗体检测率达95%的指标;孕产妇HIV抗体检测阳性率平均为0.22‰;孕期接受阻断措施相对较高(92.1%),非常有效地控制了HIV母婴传播的发生;而产妇接受阻断措施相对较低(73.0%);新生儿规范服用阻断药物者86.8%,实行人工喂养措施者97.4%;满18月龄婴儿检测11人中HIV抗体阳性2人,阻断率为81.8%(9/11)。[结论]加强艾滋病知识宣传教育,增强HIV抗体阳性孕产妇及其家属对艾滋病危害的认识,提高落实预防HIV母婴传播措施的主动性,是控制HIV母婴传播的关键。[Objective] To understand the situation of the HIV-testing among pregnant women and the situation of the effectiveness of health education program to improve PMTCT knowledge among the HIV positive pregnant woman in a large area, control the mother-to-child transmission （MTCT） of HIV occurrence, and to evaluate its effectiveness. [Methods] The data ahd information of prevention of mother-to-child transmission of HIV services, from April 2003 in 8 counties/cities of Zhoukou where more than 300 people were infected with HIV, were investigated. 0,1 this basis, analyzed the pregnant woman HIV immune body examination to complete the situation, the preventive intervention measure realization situation and the effect. [ Results] 8 counties/cities were basically able complete examination rate of 95% targets of the pregnant woman HIV immune body according to request; the positive rate of pregnant woman HIV immune body examination was equally 0.22 ‰ ; the ratio of the gestation period accepted blocks measure was high （92.1%）, which controlled the incidence of mother to child transmission of HIV very effectively; but the ratio of the parturient woman accepted blocks measure was relatively low （73.0%）; the newborn who took standard blocks medicine 86.8%, implement rate of artificial feeding measure was 97.4% ; in 11 infants aged 18 months, 2 infants＇ HIV antibody was positive, the blocking rate was 81.8%. [Conclusion] Strengthening publicity and education on AIDS knowledge to enhance HIV-positive pregnant women and their families＇ awareness against the AIDS and improving measures to prevent mother-to-child transmission of HIV are the keys to prevent HIV inother-to-ehild transmission.
