正常足月分娩产妇羊水中组织因子的测定及临床分析

摘　　要：目的通过测定正常足月产妇羊水中的组织因子（tissue factor，TF），探讨TF这一凝血反应启动因子的含量与妊娠高凝状态及羊水栓塞（amniotic fluid embolism，AFE）的关系。方法用ELISA方法分别测定正常足月妊娠（16例）血浆、羊水、羊水上清液及羊水沉渣中TF抗原水平。结果正常足月妊娠血浆、羊水、羊水上清液、羊水沉渣中TF抗原水平分别为31±9，404±186，348±177，1360±639ng／L，三种不同成分的羊水TF抗原水平与血浆比较，差异均有统计学显著性意义（P〈0．01），尤其羊水沉渣与血浆比较，差异有统计学极显著性意义∽〈0．01）。羊水沉渣中TF抗原水平与未离心羊水、羊水上清液比较，差异也有统计学显著性意义（P〈0．01）。结论正常足月产妇羊水中有大量的TF，羊水有形成分中TF抗原水平最高，羊水中的高TF水平与羊水栓塞这一产妇分娩时的严重并发症及其引发的弥漫性血管内凝血（DIC）密切相关，可能是羊水栓塞的启动因子。Objective To quantitively determine the tissue factor （TF） in the amniotic fluid of normal term parturition,and study the relations between TF that was the initial factor of the coagulation cascade and high coagulation statu with amniotic fluid embolism（AFE）. Methods Tissue factor antigen levels in amniotic fluid,blood plasma,centrifugal supernatant and sediment of 16 normal term parturition were detected with immunoenzymatic method （ELISA）. Results Tissue factor antigen levels in the blood plasma,the whole amniotic fluid,the centrifugal supernatant and the sediment were 31±9, 404±186,348± 177 and 1 360±639 ng/L,respectively. Tissue factor antigen level in three states of the amniotic sediment and in the blood plasma had obvious differences （P〈0.01）. Conclusion High concentration of tissue factor antigen is present in the amniotic fluid of normal term pregnancy. The concentration of tissue factor antigen of the cell enriched amniotic sediment is the highest in the whole amniotic fluid. Tissue factor in the amniotic fluid may be the trigger of disseminated intravascular coagulation（DIC） of amniotic fluid embolism.
