摘 要:目的探讨糖尿病病足(DFD)患者血浆同型半胱氨酸(HCY)、内皮素1(ET-1)、一氧化氮(NO)及高敏C反应蛋白(hs—CRP)水平与股动脉内膜中层厚度(IMT)的相关性。方法应用微粒子发光酶免疫分析法(MLIA)、放射免疫法(RIA)、酶联免疫吸附法(ELISA)及乳胶增强速率散射比浊法分别测定正常对照组、2型糖尿病(T2DM)正常足组和T2DM病足组患者血浆HCY,ET-1,NO及hs—CRP水平,采用彩色多普勒超声仪测定股动脉IMT值。结果T2DM病足组血浆HCY,ET-1,hs—CRP水平和股动脉IMT值显著高于T2DM正常足组和正常对照组(均P〈0.01),而NO则显著低于T2DM正常足组和正常对照组(均P〈0.01),相关分析显示,血浆HCY,ET-1及hs—CRP水平与股动脉IMT值呈显著正相关(r分别为:0.710,0.830,0.875,P〈0.01)。NO水平与股动脉IMT值呈显著负相关(r:-0.690,P〈0.01)。结论T2DM患者血浆HCY,ET-1,NO及hs—CRP水平与股动脉IMT密切相关,DFD患者血浆HCY,EF-1及hs—CRP水平异常增高是可能导致下肢动脉粥样硬化(AS)的危险因素。Objective To investigate the correlation between the levels of homocysteine(HCY),endothelin-1( ET-1),nitric oxide(NO),high sensitive C-reactive protein (bs-CRP) and the femoral artery intima-media thickness (IMT) of patients with diabetic foot disease (DFD). Methods The control group,the normal foot group and the foot disease group of T2DM patients were detected by microparticle luminesence immunoassay (MLIA), radioimmunoassay (RIA), enzyme linked immunosorbent assay (ELISA). Latex enhanced rate nephelometry transmission turbidity with HCY,ET-1 ,NO and hs-CRP of plasma respectively and the groups were studied by color doppler ultrasound with the femoral artery IMT. Results The expression of HCY,ET-1,hs-CRP and the femoral artery IMT was higher than control group and normal foot disease group with T2DM significantly (P〈0. 01) ,and the expression of NO lower than the normal foot and normal control group significantly (P〈0. 01). The related analysis showed that the levels of HCY,ET-1 and hs-CRP were correlated with the femoral artery IMT significantly (r: 0. 710,0. 830,0. 875 ,respectively,P〈0. 01) ,and there was significant negative correlation between NO and the femoral artery IMT (r: -0. 690,P〈0.01). Conclusion The levels of HCY,ET-1 ,NO and hsCRP of T2DM patients are correlated with the femoral artery IMT,and the abnormal increase of HCY,ET-1 ,NO and hsCRP of DFD patients would be one risk factor of lower limb atherosclerosis.
