摘　　要：目的分析老年人急性阑尾炎的CT特征,探讨CT诊断老年人急性阑尾炎的临床价值。方法搜集经手术和病理证实为老年人阑尾炎者32例,回顾性分析其CT表现特点。结果32例中8例急性单纯性阑尾炎;12例阑尾炎并阑尾周围炎;10例阑尾穿孔并阑尾周围脓肿;5例阑尾炎性肿块;18例阑尾或阑尾脓肿内有结石;6例并发回盲部及升结肠癌。10例并有盲肠、乙状结肠及回肠肠管壁局限性增厚。结论老年人阑尾炎症状和体征不典型,临床诊断困难,易误诊。CT对老年人急性阑尾炎及其并发症的诊断与鉴别诊断具有重要的价值。Objective To analyze the CT features of acute appendicitis in the elderly and to explore the clinical value of CT diagnosis of appendicitis in the elderly.Methods Collected 32 cases of appendicitis in the elderly confirmed by surgery and pathology,analyze retrospectively the manifestation characteristics of CT.Result of the 32 cases,there were 8 cases of simple acute appendicitis ; 12 cases of appendicitis and peri--appendicitis; 10 cases of perforated appendicitis and abscess around the appendix;5 cases of appendicitis mass; 18 cases of appendix Appendectomy or abscess with stones.6 cases of Complicated and ceCum or colon cancer. And I0 cases of appececum, sigmoid colon and ileum limitations of the intestinal wall thickening.Conclusion The clinical diagnosis of appendicitis in the elderly is very difficult when patients present with atypical signs and symptoms.CT has important significance to the diagnosis and the differential diagnosis of appendicitis and its complications in the elderly.
