疝环充填式无张力修补术在腹股沟复发性斜疝治疗中的应用

摘　　要：目的探讨疝环充填式无张力疝修补术在腹股沟复发性斜疝治疗中的应用价值。方法本研究于2009年10月至2011年10月采用疝环充填式无张力疝修补术治疗腹股沟复发性斜疝患者32例(研究组),并与采用传统疝修补术治疗的患者32例(对照组)在手术时间、住院时间、术后早期疼痛发生率、并发症发生率及复发率等方面进行比较。结果研究组患者的术后住院时间及手术时间均较对照组明显缩短,差异有统计学意义(t=5.12、12.02,P均<0.05)。研究组患者术后早期发生明显疼痛3例,占9.38%,对照组患者术后早期发生明显疼痛10例,占31.25%,研究组的术后早期明显疼痛发生率明显低于对照组,差异具有统计学意义(χ2=4.73,P<0.05)。研究组的术后并发症发生率(6.25%)明显低于对照组(25.00%),差异具有统计学意义(χ2=4.27,P<0.05);另外,研究组的术后复发率(0.00%)明显低于对照组(12.50%),差异具有统计学意义(χ2=4.27,P<0.05)。结论疝环充填式无张力修补术治疗复发性腹股沟疝的临床效果明显优于传统疝修补术,避免了传统术式破坏腹股沟区的正常解剖结构的缺点,且无张力,更加符合现代疝修补理念,具有操作简单、创伤小、复发率低、恢复快等优点,是治疗腹股沟复发性斜疝的理想术式。Objective To evaluate the clinical value of mesh plug tension-free hernioplasty for recurrent indirect inguinal hernia.Methods The patients with recurrent indirect inguinal hernia who underwent mesh plug tension-free hernioplasty from October 2009 to October 2011 were included in the study,and the comparative analysis of operating time,hospital stay,rate of early postoperative pain,complication rate and recurrent rate was made between the mesh plug repair group and the traditional repair group.Results The operation time and postoperative hospital stay in the mesh-plug group was significantly shorter than in the traditional repair group,and the difference was statistically significant(t=5.12,12.02,both P<0.05).3 cases of significant pain were found in the study group in early postoperative period,accounting for 9.38%,and 10 cases in the control group,accounting for 31.25%,the rate of early postoperative pain in the study group was significantly lower than in the control group according to statistic analysis(χ2=4.73,P<0.05).The rate of postoperative complications in the study group(6.25%) was significantly lower than in the control group(25.00%),and the difference was statistically significant(χ2=4.27,P<0.05);Additionally,the recurrence rate in the study group(0.00%)was significantly lower than in the control group(12.50%),and the difference was statistically significant(χ2=4.27,P<0.05).Conclusions Mesh plug tension-free hernioplasty has more advantage in the treatment for recurrent inguinal hernia than the traditional hernia repair,and it can avoid destroying the normal anatomical structure of the inguinal area.Moreover,tension-free is more accord with the modern hernia repair conception,and mesh plug tension-free hernioplasty is an ideal operation for recurrent inguinal indirect hernia for the advantages of simple procedure,minimal invasion,low recurrence rate,rapid recovery.
