摘 要:目的探讨血浆氨基酸浓度判别慢性乙型肝炎患者肝组织病理状态的效能。方法 148例经肝组织病理学检查的慢性乙型肝炎患者入选本研究。采用Agilent 1100系列高效液相色谱仪测定血浆氨基酸浓度。血浆氨基酸浓度判别肝组织病理状态的效能评价采用Bayes逐步判别分析。结果根据相关分析和方差分析,血浆谷氨酸、丝氨酸、苏氨酸、酪氨酸、蛋氨酸、精氨酸浓度可能对病理学分级有判别意义,血浆丝氨酸、酪氨酸浓度可能对病理学分期有判别意义。根据判别分析,只有苏氨酸、酪氨酸浓度符合模型纳入变量、进入判别病理学分级的判别函数,基于苏氨酸、酪氨酸浓度的判别函数判别G1、G2、G3的符合率分别为55.9%、44.1%、52.7%;没有1种氨基酸浓度符合模型纳入变量、进入判别病理学分期的判别函数。结论基于血浆苏氨酸、酪氨酸浓度的判别函数对病理学分级有一定判别意义,血浆氨基酸浓度对病理学分期没有判别意义。Objective To explore the efficacy of plasma concentrations of amino acids for discrimination of pathological status of liver tissues in patients with chronic hepatitis B.Methods 148 patients with chronic hepatitis B with pathological diagnoses of liver tissue were enrolled in present study.Plasma concentrations of amino acids were determined by Agilent 1100 series high performance liquid chromatography,the standard amino acid merchandises and the derivatization reagents was purchased from Agilent technologies,Ltd.Bayes stepwise discriminant analyses were used for appraisement of plasma concentrations of amino acids for discrimination of pathological status of liver tissues.Results According to the analyses of correlation and variance,plasma concentrations of glutamic acid,serine,threonine,tyrosine,methionine,arginine could have discriminant significance for pathological grading;and plasma concentrations of serine,tyrosine could have discriminant significance for pathological staging.According to the discriminant analyses,only plasma concentrations of threonine,tyrosine conformed with the entry criteria of the independents and entered the discriminant functions for discrimination of pathological grading,the correctly classified rates of the original grouped cases by the discriminant functions were 55.9% for G1,44.1% for G2,52.7% for G3,respectively;no plasma concentrations of amino acids conformed with the entry criteria of the independents and entered the discriminant functions for discrimination of pathological staging.Conclusions There could be discriminant significance for pathological grading of the discriminant functions based on the plasma concentrations of threonine,tyrosine;and there could not be discriminant significance for pathological staging of plasma concentrations of amino acids.
