甲状腺结节患者的诊断和外科治疗研究

摘　　要：目的分析研究普通外科中甲状腺结节的临床诊断和外科治疗手段。方法回顾性分析2009年1月至2011年1月期间在我院治疗的52例甲状腺结节患者的临床资料。将52例患者随机分为观察组和对照组各26例,观察组采用小切口电凝刮切法治疗,对照组采用常规手术治疗,观察记录两组患者的临床效果。结果观察组26例患者手术均获成功,手术时间30～105min,平均(50±7.5)min,术中出血25～l35ml,平均(45±3.5)ml。对照组26例患者中,3例复发,给予二次手术,手术时间95～180min,平均(120±10.5)min,术中出血100～200ml,平均(122±5.6)ml。两组临床效果比较差异无统计学意义(P>0.05);与对照组比较,观察组手术时间、术中出血量及复发率明显减少,两组比较差异有统计学意义(P<0.05);所有患者术后无一例死亡,无切口感染,无术后大出血、窒息及气管切开。结论电凝刮切法行小切口甲状腺切除术治疗安全有效。Objective To investigate the clinical diagnosis and surgical treatment of thyroid nodule in general surgery.Methods The clinical data of 52 patients with thyroid nodule which were received by our hospital from January 2009 to January 2011 were retrospectively analyzed.All patients were randomly divided into two groups,the observation group and the control group,each in 26 cases.The patients in control group were treated with common operation;the patients in observation group were treated with small cut electric coagulation scrape,clinical effect were observed.with two groups.Results The patients in observation group were doing well,with the operation time was(50 ± 7.5) minutes,with the amount of bleeding were(45 ± 3.5) ml,the patients in control group were relapse in 3 cases,with the operation time was(120 ±10.5) minutes,with the amount of bleeding were(122 ± 5.6) ml,there were no differences with clinical effect between the two groups(P > 0.05),there were significant differences with operation time,amount of bleeding and relapse rate between the two groups(P < 0.05).There was no death,infection of incisional wound,postoperative bleeding,choking,and tracheotomy ocurring in all patients.Conclusion In response to the diagnosis of thyroid nodules,carefully inquiry and recording case history,routine physical examination and laboratory examinations and so on were needed.small-cut electric coagulation scrape surgical is safety and valid.
