多次分期切除皮肤良性病变的临床观察

摘　　要：目的总结分次切除治疗皮肤良性病变如良性肿物、表浅性瘢痕的手术经验体会。方法在皮肤良性肿物或者瘢痕范围内进行部分切除,分次(间隔3～6月)将病灶缩小,直至病变完全切除;手术中和末次手术后根据情况采用不同的皮下和真皮减张处理,末次手术后酌情应用抗瘢痕治疗。结果对41例皮肤良性病变完成了分次切除手术,其中29例为面颈部、头皮部及小腿部的色素痣、疣状痣;12例为面部、四肢的瘢痕。根据病变大小不同分2～5次完成切除,术后效果良好,术后6～12月随访局部继发瘢痕不明显。结论根据不同皮肤的良性病变采用分次切除手术治疗具有简单、快捷、方便的优点,一定程度上可以减少植皮或皮瓣等其他复杂修复方法的应用。Objective To summarize the experience of the treatment of benign skin tumor(pigmented nevus,verrucoid nevus) and superficial scar by fractional resection.Methods Within the limits of the focus,the tumor was partly removed and the scar was narrowed by fractional resection.The interval was 3 to 6 months.Results 29 cases of pigmented nevus and verrucoid nevus on the face,neck,scalp and lower leg and 12 cases of scar on the face and extremities were treated by 2 to 5 times resection.The results were satisfactory.Conclusion Fractional resection is a simple,effective surgical technique for benign skin tumor(pigmented nevus,verrucoid nevus) and scar treatment with satisfied result.It is suitable for the patients who can't accept the repair by skin graft or flap and other methods,or have no enough time to receive them.
