抗核小体抗体测定对系统性红斑狼疮的诊断价值的研究

摘　　要：目的测定系统性红斑狼疮(SLE)及其它风湿性疾病患者的抗核小体抗体(AnuA)浓度,探讨AnuA对SLE的诊断价值。方法采用酶联免疫吸附试验(ELISA)测定SLE患者和疾病对照组(其他风湿性疾病)患者中的AnuA。结果(1)An-uA在SLE患者组、疾病对照组和正常人中的阳性率分别为78.6%、6.3%、0%,SLE患者AnuA阳性率显著高于疾病对照组(P<0.01)。(2)AnuA、抗dsDNA抗体、抗Sm抗体在SLE患者中的敏感性和特异性分别为78.6%、52%、36%;93.7%、91.2%、100%。AnuA敏感性明显高于其他抗体(P<0.01),特异性比较无显著性差异(P>0.05)。AnuA在抗dsDNA抗体、抗Sm抗体阴性SLE患者中阳性检出率分别为58.3%、64.5%。(3)AnuA阳性患者与阴性患者比较蛋白尿、红细胞沉降率(ESR)明显升高(P<0.05),白细胞(WBC)、血小板(PLT)、补体C3、C4明显降低(P<0.05)。结论AnuA对SLE诊断的敏感性高、特异性强;它与SLE疾病活动性密切相关,AnuA测定对SLE的诊断具有重要的临床价值,可作为SLE的又一标记性抗体。Objective To explore the clinical significance of anti-nucleosomes antibody(AnuA) in diaynosing systemic lupus erythematosus(SLE).Methods The enzyme linked immunosorbent assay(ELISA) was used to determine the concentration of AnuA in patients with SLE and controls,and compared it with other markers of SLE.Results(1) Positive percentages of AnuA in the cases with SLE,controls with other rheumatism and healthy adults were 78.6%,6.3% and 0%,respectively.There were significantly difference between the cases with SLE and controls with other rheumatism(P<0.01).(2) In the cases with SLE,sensitivity of AnuA,anti-dsDNA antibody and anti-sm antibody were 78.6%,52% and 36% while Specificity were 93.7%,91.2% and 100% respectively.Sensitivity of AnuA was obvious higher than the other antibodies(P<0.01).However,specificity was no obvious difference among them(P>0.05).Positive percentages of AnuA were 58.3% and 64.5% in the anti-dsDNA antibody and anti-sm antibody negative SLE patients,respectively.(3)Uinous protein and ESR increased in the AnuA positive patients compared with AnuA negative patients(P<0.05) while WBC、PLT、complement C3 and C4 decreasied(P<0.05).Conclusion AnuA detecting have important clinical significance for SLE,and AnuA was a new diagnostic marker of SLE,and sensitivity and specificity of AnuA were better than other diagnostic marker of SLE.
