摘  要:目的探讨尿蛋白阴性糖尿病患者随机尿中三种微量蛋白和NAG酶联合检测在临床诊断早期肾损伤的应用价值。方法测定73例2型糖尿病患者和40例健康查体者尿中微量白蛋白(MALB)、转铁蛋白(TF)、β2-微球蛋白(β2-MG)和N-乙酰-β-D-氨基葡萄糖苷酶(NAG),同时检测尿肌酐作对比,保证结果准确性。三种微量蛋白采用免疫比浊法,NAG采用终点法。结果糖尿病患者TF、MALB、β2-MG、NAG明显高于正常对照组(P<0.01);四项联合检测阳性率达93.1%,明显高于单项检测。结论TF、MALB、β-MG、NAG检测是诊断糖尿病早期肾损伤的敏感指标,肾小管损伤早于肾小球。Objective To study the clinical value of three urinary trace proteins and NAG in the diagnosis of earlier diabetic nephropathy(DN) of DM whose urinary protein is negtive. Method Urinary mircroalbumin、transferrin、β_2-microglobumin and N-acety1-β-D-glucosmnccharase in 73 patients with type 2 diabetes and 40 health subjects were measured by immunitytuibidimetry assay and end-point method assay and compared with their urinary critinine,to assure veracity. Result Campared with health control group,in DM group urinary three trace proteins and NAG are patently higher(P<0.01),and their detennination together is significantly higher than single one(93.1%)。 Conclusion Urinary TF、MALB、β_2-MG and NAG are sensitive items;Detemination together increase greatly the diaganositic rate of DN;The damage of glomemlus is earlier than that of glomemlus.
