补阳还五汤对脑梗死患者胰岛素及内源性多肽的影响

摘　　要：目的 检测脑梗死患者血胰岛素及内源性多肽的含量,探讨补阳还五汤治疗脑梗死的机制。方法 应用补阳还五汤治疗27例脑梗死患者(治疗Ⅰ组),22患者为对照组(治疗Ⅱ组),20例健康人为健康对照组,比较两组神经功能恢复情况及三组的血清胰岛素(INS)、血浆β-内啡肽(β-EP)、神经降压素(NT)、神经肽Y(NPY)的含量。结果 治疗Ⅰ组总有效率为88.9％,治疗Ⅱ组总有效率为59.1％,两组疗效比较差异有非常显著性(P<0.01);脑梗死患者血浆INS、NPY和β-EP含量高于健康人组(P<0.01),而NT含量则较之为低(P<0.01);治疗Ⅰ组以补阳还五汤治疗后INS、NPY和β-EP含量明显降低;而NT含量升高,前后差异有显著性(P<0.01),治疗Ⅱ组NPY和β-EP含量有所降低(P<0.05),其余指标则改变不明显。结论 补阳还五汤对脑梗死患者神经功能缺损的恢复具有促进作用,其机制可能与该方能有效地改善脑缺血区域的血液循环及代谢障碍程度,调节NPY、β-EP和NT的含量有关。Objective To explore the mechanism of "Bu Yang Huari Wu" decoction in treating cerebral infarction. Methods Fifty - three patients were divided into two groups, 27 cases of cerebral infarction were treated with "Bu Yang Huan Wu" decoction (treatment group I ), 26 cases were treated with conventional therapy (treatment group Ⅱ) and 20 people of health ( normal group) as control. The improvement of neural function, level of blood serum insulin (INS), plasma β- endorphin (β-EP), neuropeptide Y (NPY), neurotensin (NT), norepinephrine (NE) were examined in three groups.Results The total effective rate was 88.9% in treatment group I and 59.1% in treatment group Ⅱ respectively, P < 0.01 .The concentration of INS, β- EP and NPY in two groups of cerebral infarction were obviously higher than those of normal group, NT was obviously lower than of normal subjects, all P < 0.01 .The result showed that the concentration of INS, β-EP and NPY were decreased and NT was increased in treatment group I after taking decoction, all P < 0.01; NPY and β-EP were decreased in treatment group II ( P < 0.05), but no obvious change in other data.Conclusion These data suggest that "Bu Yang Huan Wu" decoction can promote recovery of neural function, the mechanism might be related with ameliorating blood circulation and degree of metabolism disturbance in regions of cerebral ischemia effectively, adjusting contents of NPY, β-EP and NT.
