摘 要:目的:探讨烧伤后血浆白细胞介素8(IL-8)、血管细胞粘附分子1(VCAM-1)变化与烧伤严重程度及呼吸功能障碍的关系。方法:对18例TBSA为35%~97%的烧伤患者,分别于伤后12h,1,2,4,7d取血,分离血浆,采用ELISA法测定1L-8和VACM-1的水平。结果:TBSA<50%组伤后血浆IL-8水平持续升高,于伤后第4天达峰值。TBSA>50%组各时相点水平明显高于TBSA<50%组,两组相比差异显著(P<0.05)。出现肺功能障碍组IL-8水平明显高于无肺功能障碍组。TBSA>50%组和肺功能障碍组VCAM-1水平明显高于正常对照组、TBSA<50%组及无肺功能障碍组(P<0.05)。结论:热损伤后IL-8与VCAM-1能影响全身免疫反应,并与肺功能障碍的发生、发展有关。烧伤面积大小。Objective: To observe the alterations of plasma levels of IL-8 and VACM-1 in different burn area patients and their roles in burns accompanied by lung dysfunction. Methods: The plasma was separated from blood samples of 18 patients 12 h and 1,2,4, 7 d postburn (PB). The IL8 and VACM1 in plasma were detected by ELISA kit (Genzyme). Results: The plasma level of IL8 in TBSA<50% group increased continuously and reached at a peak in PB 4 d. The IL-8 level of TBSA≥50% group had an abrupt elevation and was significantly higher than those of TBSA<50% group in PB 4, 7 d(P<0.01). The plasma level of IL-8 in lung dysfunction group was also significantly higher than that in normal lung function group in PB 4,7 d(P<0.01). The changes of plasma VACM-1 were similar to those of IL-8 in each group. Conclusion: The alterations of plasma IL-8 and VACM levels are influenced by severity of burn and dysfunction of lung in thermal injuries.
