测定糖化血清蛋白新型标准参照物果糖哌啶的临床应用研究

摘　　要：测定糖化血清蛋白的含量，是诊断糖尿病的重要指标之一。糖化血清蛋白是血液中葡萄糖与血清蛋白通过非酶化作用形成一种具有酮胺结构的物质，该物质在碱性条件下能与氯化硝基四氮唑蓝（ＮＢＴ）发生还原反应而呈色，通过比色方法，测其吸光度，并用以计算糖化血清蛋白的含量。据此，作者合成了具有酮胺结构的１－脱氧１－哌啶－Ｄ－果糖（ＤＰＦ），并首次作为测定糖化血清蛋白的标准参照物用于临床检测。本文简介了ＤＰＦ的合成方法和结构表征；初步考查了ＤＰＦ与ＮＢＴ反应有良好的线性关系，同时，ＤＰＦ、血清与ＮＢＴ反应有相类同的吸收光谱，最大吸光度都在５３０—５５０ｎｍ；着重介绍了测定糖化血清蛋白的方法和临床检测情况。经５０例健康人和３Ｏ例糖尿病人临床检验，测其糖化血清蛋白含量，正常人为１．５０±０．３１ｍｍｏｌ／Ｌ，糖尿病人为２．５２±０．７２ｍｍｏｌ／Ｌ（ｘ±ｓ），两组没有交叉现象，有显著性差异（ｐ＜０．０５）。初步实验结果表明，ＤＰＦ可作为测定糖化血清蛋白的标准参照物。This article iniroduces briefly the synthesis of 1 deoxy 1 piperi-dine D fructose(DPF) as a new standard reference material to measure glyeosylatedserum protein,to examine preliminarily the good linear relationship of DPF reacting withNitro Blue Teterazolium Chloride(NBT) and the similar absorbing spectrum of DPF andserum reacting with NBT respectively with the maximum absorbance of both at 530 550nm, this article emphasises on describing the method of mearsuring glycosylatedserum protein and its clinical inspection,The clinical inspection on 50 cases of normaladults and 30 cases of diabetic patients showed that the measurement of glycosylatedserum protein was 1.50±0. 31 mmol/L in the former and 2.52±0.72mmol/L(x±s) inthe latter, No intersection between the two groups was found;a clear difference hadshown with P<0.05. The preliniinary experiment showed that DPF could be used as astandard reference material to measure glycosylated serum protein.
