心缓康胶囊改善窦房结功能的研究

摘　　要：目的：探讨心缓康胶囊对窦房结功能的影响。方法：１３１例窦房结功能障碍（ＳＮＤ）患者随机分为治疗组１０１例和对照组３０例，分别服心缓康胶囊及心宝１个月。对比２组患者服药前后的临床症状、平静心率、固有心率、动态心电图（ｄＥＣＧ）和心脏电生理改变。结果：治疗组服药前后的平静心率分别为４７次／ｍｉｎ和５８次／ｍｉｎ；固有心率分别为７４次／ｍｉｎ和９０次／ｍｉｎ；ｄＥＣＧ最慢心率分别为４１次／ｍｉｎ和４９次／ｍｉｎ；ｄＥＣＧ平均心率分别为５７次／ｍｉｎ和６３次／ｍｉｎ；ｄＥＣＧ２４小时总心跳数分别为７０５２９次／ｄ和８９７６７次／ｄ；窦房结恢复时间（ＳＮＲＴ）分别为１７７１ｍｓ和１５０５ｍｓ；窦房结传导时间（ＳＡＣＴ）分别为１６０ｍｓ和１３９ｍｓ；文氏点分别为１３１次／ｍｉｎ和１４２次／ｍｉｎ（Ｐ均＜０．０００１），其中５项指标的改善较对照组明显（Ｐ＜０．０１或Ｐ＜０．００１）。服药后治疗组并发的各种心律失常较用药前明显减少，未见明显副作用。结论：心缓康胶囊是治疗ＳＮＤ的较有效的中药制剂，它除可通过植物神经调节窦房结功能外，可能还有直接促进窦房结功能恢复的作用。Objective:To explore the effect of “xin huan kang” capsule on sinoatrial node function.Methods:One hundred and thirtyone cases of sinoatrial node dysfunction (SND) were divided randomly into treatment group (n=101) and control group (n=30).The treatment group was treated with oral “xin huan kang” capsule for 30 days,while the control group was treated with oral “xin bao” for 30 days.Then,to observe and compare the changes of symptom,resting heart rate (RHR),intrinsic heart rate (IHR),dynamic electrocardiography and cardiac electrophysiology before and after therapy between 2 groups.Results:In the treatment group,the RHR were 47 beats/min and 58 beats/min;the IHR were 74 beats/min and 90 beats/min;the minimal 24 hours beats were 70 529 and 89 767 ,sinus node recovery time (SNRT)was 1 771 ms and 1 505 ms;the sinoatrial node conduction time(SACT) was 160 ms and 139 ms and the Wenckeback′s block point was 131 beats/min and 142 beats/min before and after therapy respectively (all P<0000 1).There were 5 parameters among above mentioned improvements were greater as compared with that achieved in control group (P<001 or P<0001).In addition,arrhythmia events complicated by SND were decreased significantly after therapy in treatment group and no obvious sideeffects were found in both groups.Conclusions:“Xin huan kang” capsule is an effective Chinese herbal fomula for treating SND.It can regulate and enhance sinoatrial node function through autonomic nerves and may also have a direct enhancement effect on sinoatrial node function.
