中药“强心基本方”治疗老年难治性心力衰竭疗效观察

摘　　要：目的：探索中药“强心基本方”治疗老年难治性心力衰竭（心衰）的疗效。方法：９２例住院老年难治性心衰患者随机分为常规抗心衰治疗（Ａ组，３０例）、中药“强心基本方”结合常规治疗（Ｂ组，３２例）及纯中药“强心基本方”治疗（Ｃ组，３０例）３组，观察疗效并比较治疗前后每搏输出量（ＳＶ）、射血分数（ＥＦ）、动脉血氧分压（ＰａＯ２）、二氧化碳分压（ＰａＣＯ２）及血液流变学指标变化。结果：Ｂ组显效率为４０．６％，总有效率为８７．５％，均高于Ａ组（分别为１６．７％和６３．３％）及Ｃ组（分别为１３．３％和６３．３％），均有显著性差异（Ｐ均＜０．０５）；Ｂ组ＳＶ、ＥＦ和ＰａＯ２提高程度及血液粘度降低值均较Ａ和Ｃ组显著；并可减少抗心衰药物的不良反应。结论：中药“强心基本方”具良好抗心衰效应，与其它抗心衰药物同用有协同作用，尤为适宜老年心衰患者的治疗。Objective:To explore the efficacy of cardiotonic basic traditional Chinese medicine (BTCM) in treatment of senile refractory heart failure (RHF).Methods:Ninetytwo hospitalized patients of senile RHF were divided randomly into 3 groups:group A(n=30) was treated with conventional scheme;group B(n=32) was treated with cardiotonic BTCM combined use of routine method;group C (n=30) was treated with cardiotonic BTCM alone.In addition,to observe and compare the therapeutic efficacy,stroke volume (SV),ejection fraction (EF),partial pressure of oxygen in artery (PaO2),partial pressure of carbon dioxide in artery (PaCO2) and hemorrheology changes before and after therapy.Results:The obvious effective rate and total effective rate in group B were 406% and 875% respectively,it were higher than those in group A (167% and 633%) and group C(133% and 633%),there was obvious difference between group B and group A,C (all P<005).On the other hand,the raise extent of SV,EF,PaO2 and decrease blood viscosity in group B were superior to those in group A,C.The sideeffect of cardiotonic medicine was decreased in group B.Conclusions:Cardiotonic BTCM treating heart failure was possessed of better effect,it has a synergetic action with other antiheart failure drugs.Moreover,cardiotonic BTCM is a reliable therapeutic agent in particular for senile patients.
