摘  要:目的了解2型糖尿病肾脏疾病患者血清内脂素和脂联素水平的变化,并探讨其与2型糖尿病肾脏疾病及各项代谢指标之间的关联。方法选取2型糖尿病肾脏疾病患者52例(DKD组),46例单纯2型糖尿病患者(T2DM组)及40例正常对照人群(对照组),采用酶联免疫吸附测定(ELISA)法检测内脂素水平,用放射免疫分析法(RIA)测定脂联素和胰岛素,另外测量体质量、身高、腰围、体质量指数、腰臀比及各项生化指标,并分析内脂素、脂联素与体质量指数、腰臀比、血糖、糖化血红蛋白、空腹胰岛素、胰岛素抵抗指数(HOMA-IR)等的关系。结果DKD组及T2DM组内脂素水平明显低于对照组,分别为(11.58±7.32)μg/L,(12.04±6.17)μg/L vs(16.89±5.76)μg/L(P<0.05),而DKD组与T2DM组之间无明显关联(P>0.05);而脂联素水平明显降低,分别为(3.64±0.70)mg/L vs(6.91±3.26)mg/L,(4.82±1.86)mg/L vs(6.91±3.26)mg/L(P<0.01),DKD组与T2DM组相比差异有统计学意义(P<0.01);DKD组患者血清内脂素水平与腰臀比呈正相关(r=0.406,P<0.01);脂联素与HOMA-IR、体质量指数、腰臀比、甘油三酯、低密度脂蛋白胆固醇、空腹胰岛素、空腹血糖水平呈显著负相关,相关系数分别为-0.612、-0.431、-0.761、-0.623、-0.380、-0.332和-0.393(均P<0.01)。结论2型糖尿病肾脏疾病患者血清内脂素及脂联素水平下降,且内脂素与脂联素是糖尿病肾脏疾病发生发展的独立危险因素。Objective To investigate the change of visfatin and adiponectin levels in patients with type 2 diabetic kidney disease(DKD) ,and their relation with the metabolic parameters. Methods Fifty-two patients with DKD(DKD group) ,46 subjects with type 2 diabetes mellitus(T2DM group) and 40 healthy controls(control group) were collected, then the body mass, body height, waist circumference, body mass index(BMI), wait-hip ratio (WHR) and the fasting levels of glucose, insulin, adiponectin and visfatin measured. The visfatin was measured with enzyme-linked immunoassay(EIA) ,the adiponectin and insulin levels with radioimmunoassy(RIA). The relationship of serum visfatin and adiponeetin levels with BMI, WHR, fasting plasma glucose (FPG), HbA1c, fasting insulin (FINS) levels and HOMA-IR were analyzed. Results The visfatin levels of DKD group and T2DM group decreased significantly than those of control group, (11.58±7.32) μg/L, (12.04±6.17) μg/L vs (16.89±5.76) μg/L ( P 〈0.05) ,and there was no correlation between DKD group and T2DM group( P 〈0.05). The adiponeetin levels of DKD group and T2DM group were significantly lower than those of control group (3.64± 0. 70) mg/L, (4.82 ±1.86) mg/L vs (6. 91 ± 3.26) mg/L ( P 〈0. 01) ,compared with T2DM group,DKD group had significance ( P 〈0.01). The visfatin level was positively related to WHR in DKD group (r =0. 406, P 〈 0.01). There was negatively correlation between adiponectin and HOMA-IR,BMI, WHR, TG, LDL-C, FINS, FPG( r =-0. 612, -0. 431, -0. 761, -0. 623, -0. 380, -0. 332 and -0. 393,all P〈0.01). Conclusion Visfatin and adiponectin levels decrease in type 2 diabetic kidney disease patients~ visfatin and adiponectin are independent risk factors in development of type 2 diabetic kidney disease.
