中老年支原体肺炎的临床分析(附20例报告)

摘　　要：目的：对一组中老年支原体肺炎病例进行分析，以提高诊断水平减少误漏诊。材料方法：２０例患者诊断经临床表现，Ｘ线平片所见和血清间接血凝试验阳性确立。结果：患者均有发热，平均温度３８．０°±０．７°Ｃ，血白细胞升高和血沉加快者仅８例，余１２例属正常范围，体征轻微。Ｘ线胸部平片显示肺内病变为１００％，其中肺野出现斑片状阴影者１５例（７５％）。全组病例应用红霉素治疗治愈，复查胸部Ｘ线平片，肺部病灶消失。结论：中老年支原体肺炎表现更不典型，Ｘ线胸部平片发现支原体肺炎敏感，间接血凝试验阳性有确定诊断价值，大环内酯类抗生素疗效显著。Purpose:A series of patients with mycoplasmal pneumonia in middle and old age were analysed,so that we could enhance our diagnostic level and reduce misdiagnosis or missed diagnosis.Materials and Methods:The diagnosis of 20 patients were established by clinical manifestation,X ray plain film findings and seropositive of indirect agglutination test.Results:All patients had fever,average temperature was 38.0°±0 7°C.Only 8 cases had blood sedimentation accelerated and hyperleukocytosis.The signs was slight.X ray plain film of the chest depicted the lesion of intrapulmonary in 100%,in which 15 cases were patchy and spotted shadow.All patients were cured by erythromycine,and the lesions disapeared on X ray plain film of the chest.Conclusion:Manifestation of mycoplasmal pneumonia in middle and old age is more atypical.X ray plain film of the chest is a sensitive method to find mycoplasmal pneumonia.Seropositive of indirect agglutination test has the value of establishing the diagnosis.The macrolides antibiotic has excellent curative effect.
