摘 要:目的:探讨血管内皮生长因子(vascular endothelial growth factor,VEGF)在糖尿病大鼠肾皮质中的表达及PPARγ激动剂吡格列酮对其表达的影响。方法:①雄性SD大鼠经单次腹腔注射链脲佐菌素(STZ,剂量为55 mg/kg)建立糖尿病模型,将糖尿病大鼠随机分为糖尿病组(DM组)、糖尿病吡格列酮(pioglitazone 10 mg.kg-1.d-1)干预组(DP组),并以正常组(NC组)作对照。②干预8周后,观察各组大鼠24 h尿蛋白(mg/24 h)、血尿素氮(BUN)、血肌酐(Scr)、血糖及血脂的变化;用免疫组织化学和逆转录PCR检测肾皮质VEGF蛋白及mRNA表达。结果:①DM组大鼠24h尿蛋白、BUN、Scr、肾皮质VEGF蛋白及mRNA表达均较NC组明显升高;DP组大鼠24 h尿蛋白、BUN、Scr、肾皮质VEGF蛋白及mRNA表达较DM组均有降低。②DM组血糖、血脂与NC组相比,差异有统计学意义,与DP组比较差异无统计学意义。结论:糖尿病大鼠肾皮质VEGF表达增强,吡格列酮可抑制糖尿病大鼠肾皮质VEGF的表达,减少尿蛋白,改善肾功能,延缓糖尿病肾病的发生、发展,发挥非降糖的直接肾保护作用。Objective: To investigate the effect of pioglitazone on the expression of vascular endothelial growth factor in the kidney of diabetic rats. Methods: Diabetic model was established by a single injection of streptozotoein(STZ 55 mg/kg), and diabetic rats were randomly divided in to 2 groups: untreated diabetes mellitus group (DM group), pioglitazone -treated diabetes mellitus group (DP group 10mg·kg^-1·d^-1). In addition, the normal rats served as a normal control group (NC group). Eight weeks later, the twenty-four hours urine protein level, serum urea nitrogen, serum ereatinine, blood glucose and serum tipids were detected. The expressions of VEGF were detected by immunohistoehemisty and Reverse transcription-PCR. Results: Compared with NC group, the twenty-four hours urine protein level, serum urea nitrogen and serum creatinine in DM group were significantly increased, and the expression of VEGF was significantly higher. Compared with DM group, the twenty-fours hour urine protein level, serum urea nitrogen and serum creatinine were significantly decreased in DP group, while there was a significant reduction in both the VEGF and the VEGF mRNA expression after treatment group. Blood glucose and serum lipids profile were significantly changed in DM group compared with NC group, while they had no significant difference between DM group and DP group. Conclusion: These results suggest that the expression of VEGF is increased in the kidney of STZ induced diabetic rates. Pioglitazone can reduce the expression of VEGF, and the excretion of urine protein, and to ameliorate renal function, whose mechanism may be some direct nephroprotective effects.
