应用骨髓内减压术治疗成人股骨头缺血性坏死

摘　　要：本文对７２例成人股骨头缺血性坏死病人，１０６个股骨在其近端大粗隆、股骨颈、股骨头中心及其负重区，进行骨髓内压测定。结果表明：非创伤性股骨头缺血性坏死各期各区的骨髓内压均高于创伤性股骨头坏死。Ⅰ、Ⅱ期股骨头缺血性头坏死中心、头负重区压力明显高于大粗隆及股骨颈，Ⅲ、Ⅳ期股骨头中心及其负重区压力呈下降趋势，而大粗隆及股骨颈反而升高。测压后，对７２例病人大粗隆、股骨颈、股骨头中心及股骨头负重区进行分区减压术。对６２例病人随访２～４年３个月，根据修订后的８２北京髋关节疗效评定标准评价，优２６例；良２４例；可８例；差４例。此手术可以缓解疼痛，对于保护性关节活动受限也可很快得以改善。Measurement of IMP of the proximal femur,including the great trochanter,neck,center and weight bearing area of the head were performed in 106 femurs of 72 patients with ANF.The results revealed that the IMP in the four areas of every stages in non traumatic ANF were higher than that in traumatic ANF and that in Ⅰ,Ⅱ stages of ANF,the IMP of the center and weight bearing area were higher than that of the great trochanter and neck;In Ⅲ、Ⅳ stages of ANF,the IMP of the center and weight bearing area decreased,while IMP of the great trochanter and neck increased.After the measurement,fractional decompression was done in 72 patients.Follow up of 62 cases extended to 2 4 years and 3 months.Based on the 82 Beijing revised criteria of hip disease,the outcome was excellent in 26,good in 24,fair in 8 and poor in 4 patients.The opration could relieve pain rapidly,decrease the protective motion limitation,and stimulate the regeneration of the blood vessels.
