带缝匠肌肌蒂髂骨瓣植入术治疗股骨头缺血坏死55例报告  

Avascural Necrosis of the Femoral Head Treated with Sartorius Pedicle Iliac Graft:A Brief Report of 55 Cases

摘　　要：本文对我科应用带缝匠肌肌蒂髂骨瓣植入术治疗的８７例股骨头缺血坏死患者进行了追踪观察，其中随访３年以上者５５例６５个髋关节，结果按王氏疗效标准评定疗效，优良率达８６．２％。结合病情对治疗结果进行比较分析发现，该术式能较好的改善病变股骨头的血运，诱导成骨及加速坏死骨区的再骨化，适合于股骨头缺血坏死的Ⅰ。We reviewed the results of sixty-five sartorius pedicle iliac grafts that had performed for 55 patients who had suffered fromavascural necrosis of the femoral head.At mean 3.5 years postoperatively,fifty-five hips (86.2%per cen)kad a good or exellent result according to Wang Yixing hip-scoring system,and ten had a fair or poor result.Nine of the ten failures were in osteonecrosis of the femoral head which ws stage Ⅲ or Ⅳ according to the classification of Ficat.Conversly, the fifty-five successes were stage Ⅰor Ⅱ。The results of this study suggest that sartorius pedicle iliac grafting can improve blood circulation of the pathogenetic femoral head and induce bone generation as to promote ossification of the osteonecrosis area.This opration is a successful treatment for Ficat stage Ⅰ,Ⅱor Ⅲ,dease in which the femoral head didn't collapse seriously.
