摘  要:目的:探讨颈动脉、股动脉内-中膜厚度(IMT)及肱动脉流量介导的舒张功能(FMD)与冠心病的相关性。方法:经冠状动脉造影正常者29例(正常组),冠状动脉有病变者76例(冠心病组),利用高频超声检测颈动脉、股动脉IMT及肱动脉FMD。结果:单支病变组、多支病变组颈动脉、股动脉IMT明显高于正常组,肱动脉FMD则明显低于正常组(均P<0.01),单支病变组与多支病变组之间差异亦有统计学意义(P<0.01);冠状动脉病变积分(修正Gensini积分)与颈动脉、股动脉IMT呈明显正相关,与肱动脉FMD呈明显负相关(均P<0.0001)。结论:颈动脉、股动脉IMT及肱动脉FMD与冠心病存在相关性,超声检测颈动脉、股动脉IMT及肱动脉FMD可为早期发现、治疗和预防冠心病提供依据。Objective To explore the relationships between carotid, femoral artery intima-media thickness (IMT) and brachial artery flow-mediated dilation (FMD) in patients with coronary artery disease. Methods 105 patients were divided into normal group (n = 29) and coronary heart disease (CHD) group (n = 76)according to the results of coronary angiography. The brachial artery FMD as well as the carotid and femoral IMT were measured by high-resolution ultrasound. Results As compared to normal group, brachial artery FMD was significantly lower and IMT of the carotid and femoral were significantly higher in CHD group (P 〈 0.01 ). There were significant differences in brachial artery FMD and IMT of the carotid and femoral between one-vessel and multi-vessel stenosis groups (P 〈 0.01 ). The revised Gensini score was positively related to carotid and femoral IMT and was negatively related to brachial artery FMD (P 〈 0.000 1 ). Conclusion Carotid and femoral IMT and brachial artery FMD are related to coronary heart disease. Both of the detection of brachial artery FMD and IMT of carotid and femoral by ultrasound can provide evidence for discovering, treating, and prognosticating CHD.
