Roy适应模式用于机械通气患者整体化护理22例临床观察

摘　　要：目的:探讨Roy适应模式用于整体化护理机械通气(>72 h)患者的临床效果。方法:选取重症监护病房长期接受机械通气的患者40例,随机分为观察组22例和对照组18例,观察组采用Roy适应模式中的生理、自我概念、角色功能及相互依赖4种适应模式进行整体化护理,对照组采用常规护理方法。观察比较两组患者的脱机成功率、机械通气时间、住院时间及住院费用。结果:在两组患者的APACHEⅡ评分与使用抗生素情况均无显著性差异(P均>0.05)的条件下,观察组脱机成功率显著提高(P<0.05),使用呼吸机的天数明显缩短(P<0.05)。同时随访资料显示,住院天数缩短,住院费用降低,但是统计学无显著性差异(P>0.05)。结论:对长期接受机械通气患者中实行Roy适应模式能缩短机械通气的时间,提高脱机成功率。Objective ： To study the clinical effectiveness of mechanical ventilation by applying Roy adaptation model in holistic nursing care （ 〉 72 h）. Methods： Forty patients with long - term mechanical ventilation in ICU were randomly divided into two groups （ 20 cases for each group ）. Physiological, self - concept, function of role and interdependence of Roy adaptation model were applied to holistic nursing care in observation group and conventional nursing care was taken in control group, observing and comparing success rate of weaning from ventilator, time of mechanical ventilation, length of stay in hospital and costs as well. Results ： Without significant difference in comparison of APACHE Ⅱ score and the use of antibiotics （ P 〉 0.05 ） , the success rate weaning from ventilator significantly increased in observation group （P 〈 0.05） ; the days of using ventilator was significantly shortened （ P 〈 0.05 ）. At the same time follow - up data revealed that the length of stay in hospital was shortened and the costs of hospitalization reduced, but there was no significant difference statistically （P 〉 0.05）. Conclusion ： Roy adaptation model can shorten the time of mechanical ventilation and increase success rate of weaning from ventilator when the patients have long- term mechanical ventilation.
