摘  要:目的了解临床分离的产ESBLs大肠埃希菌耐药基因存在状况和菌株的耐药性关系。方法自临床分离20株产ESBLs大肠埃希菌,采用聚合酶链反应(PCR)检测β-内酰胺酶基因TEM、SHV、LEN、CTX-M-1、CTX-M-9、VEB、GES、PER。结果20株大肠埃希菌中β-内酰胺酶基因TEM、SHV、LEN、CTX-M-1、CTX-M-9、VEB、GES、PER阳性率分别为75.0%、20.0%、95.0%、15.0%、70.0%、5.0%、85.0%、70.0%,β-内酰胺类药物及含酶抑制剂药物的耐药率较高。结论临床分离的产ESBLs大肠埃希菌TEM、LEN、CTX-M-9、GES、PER基因携带率高,β-内酰胺类药物耐药率较高可能与细菌携带β-内酰胺酶耐药基因有关,产ESBLs大肠埃希菌可导致克隆传播医院感染,并存在暴发性流行。OBJECTIVE To investigate the β-1actamase genes in Escherichia coli and their resistance. METHODS Twenty strains of E. coli producing extended spectrum β-lactamases (ESBLs) were isolated from hospitalized patients. β-Lactamase drug-resistant genes were detected by PCR. RESULTS There were higher positive rates of β-1actamase genes of TEM, SHV, LEN, CTX-M-1, CTX-M-9, VEB, GES, PER (75.0%, 20.0%, 95.0%, 15.0%, 70.0%, 5.0%, 85.0% and 70.0%, respectively). This study showed that there were high positive rate of drug resistance. CONCLUSIONS There are very high positive percentages of TEM, LEN, CTX-M-9, GES, and PER genes in ESBLs producing E. coli isolated clinically, and showing highly drug-resistant E. coli of producing ESBLs can be transmitted in hospital and induce fulminant prevalence.
