摘　　要：目的:通过调查我院门诊处方抗菌药物使用状况,并就不合理用药进行分析,为临床合理用药和医院科学管理抗菌药物的使用提供可靠依据。方法:随机抽取我院2008年1～12月每月门诊处方100张,共计1200张,对门诊合理用药进行处方评价。再随机抽取2008年1～12月每月处方,共计11103张,进行处方差错分析,统计抗菌药物不合理使用情况,并调取门诊在这12个月抗菌药物消耗总量,考察注射剂、口服剂用药频度(DDDs)的排名情况。结果:门诊抗菌药物的DDDs前10位的药物:β-内酰胺类6种、氟喹诺酮类2种和大环内酯类1种,使用基本合理,但在使用中存在用药起点高、用法用量不合理等一些亟待解决的问题。结论:我院门诊抗菌药物处方基本合理,但医院仍需进一步加强抗菌药物合理使用的管理,通过处方点评等干预措施,促进合理用药,确保患者用药安全。OBJECTIVE： To investigate the utilization of antibiotics in our hospital and analyze the irrational use of antibiotics so as to provide reliable basis for rational drug use in the clinic as well as scientific management on the use of antibacterials. METHODS： A total of 1 200 outpatient prescriptions （100 monthly from January to December in 2008 ） randomly sampled in our hospital were subjected to evaluation in respect of the rational use of antibiotics. Another 11 103 outpatient prescriptions randomly sampled monthly from January to December in 2008 were analyzed in respect of prescription error and the irrational use of antibiotics. The consumption quantity of antibiotics and the order of DDDs of antibacterials （injections and oral preparations） within the 12 months were investigated. RESULTS： Six kinds of β - lactams, two kinds of quinolones and one kind of macrolides ranked among the first 10 places on the DDDs list of the antibacterial drugs used in the outpatients. The outpatient utilization of antibacterials in our hospital was basically rational, but there were some problems such as high starting point and irrational administration and dosage which remain to be tackled urgently. CONCLUSION： The outpatient utilization of antibacterials in our hospital was rational on the whole, but the management on the use of antibiotics in the hospital should further strengthened by taking intervention measures such as prescription evaluation etc so as to promote rational drug use and ensure medication safety.
