牙周炎患者龈沟液IL-10、IL-18和IFN-γ测定的临床意义

摘　　要：目的:测定牙周炎患者龈沟液IL-10、IL-18及IFN-γ浓度,探讨三种细胞因子含量的变化及其意义。方法:IL-10、IL-18及IFN-γ三种细胞因子均采用放射免疫分析。结果:牙周炎患者治疗前IL-18及IFN-γ两项龈沟液细胞因子的浓度极显著高于正常对照组(P均<0.01);IL-10水平略低于正常对照组,但无明显统计学意义(P>0.05);治疗一个月后,IL-18及IFN-γ两项龈沟液细胞因子的浓度显著下降,但仍显著高于正常对照组(P均<0.05);IL-10含量则升高极显著(P<0.01)。相关分析数据显示,牙周炎组IL-18及IFN-γ两项龈沟液细胞因子的浓度与牙周袋深度、附着丧失呈显著正相关(r=0.2617、0.2802,P均<0.05)。而IL-10浓度与牙周袋深度、附着丧失程度呈显著负相关(r=-0.1743,P<0.05)。结论:牙周炎患者龈沟液IL-10、IL-18及IFN-γ三项细胞因子水平发生了显著变化,表明牙周炎患者体内存在免疫功能调节紊乱。Objective To explore the clinical significance of changes of gingival erevicular fluid levels of interleukin - 10 （IL-10）, interleukin-18 （IL -18） and interferon -γ in patients with periodontitis. Methods Gingival erevicular fluid levels of IL - 10, IL - 18 and IFN - γ were determined with RIA in 42 patients with periodontitis both before and after trentment as well as in 30 controls. Results Before treatment, the gingival crevicular fluid level of IL - 18 and IFN - γ in the patients were significant higher than those in controls （P 〈0.01）. The gingival crevicular fluid level of IL - 10 were slightly, but not significantly lower than those in controls （ P 〉0.05）. After one month of treatment , the gingival crevigeular fluid levels of IL - 18 and IFN -γ were markedly dropped, but remained significantly higher than those in controls （P 〈0.05）. The gingival crevicular fluid levels of IL- 10 were significant higher than those in controls （ P 〈 0. 01 ）. The gingival fluid contents of IL - 10 and IL - 18 were positively correlated with the depth of periodontal pouch and looseness of attachmem （ r = 0. 2617, r = 0. 281）2, P 〈 0.05） but the interferon - γ contents were negatively correlated （ r = - 0. 1743 ,P 〈 0.05）. Conclusion The changes of gingival crevicular fluid levels of interleukin - 10, interleukin - 18 and IFN - γ in patients with periedontis suggested that them were disturbances of immunomodulation.
