顽固性创伤性硬膜下积液脑室镜手术炎症反应对比研究

摘　　要：目的研究创伤性硬膜下积液(TSH)患者脑室镜手术前后以血清、脑脊液白介素6(IL-6)、白介素8(IL-8)、肿瘤坏死因子α(TNF-α)为主要指标的全身及局部炎症变化规律。方法采用酶联免疫吸附(ELISA)法检测8例TSH患者术前后不同时间内体温,血清白细胞,脑脊液IL-6、IL-8、TNF-α水平,并随机抽取8例颅外病变术前病人进行对照。结果TSH患者脑脊液炎症因子水平升高明显(P<0.01),血清炎症因子无明显升高(P>0.05)。结论TSH患者经脑室镜术后全身炎症反应轻微,局部炎症反应加重。硬膜下腔局部炎症反应及手术干预是促进顽固性TSH吸收闭合的必要条件。Objective To investigate the changes in serum and CSF levels of IL-6, IL- 8 and TNF -α in patients with TSH （traumatic subdural hydrops）. Methods ELISA was applied for measuring serum and CSF levels of IL - 6, IL - 8 and TNF - α in 8 patients with TSH and they were compared with those of 8 patients with extracranial diseases. Results The results indicated that CSF levels of IL - 6, IL - 8 and TNF - α were significantly increased within 1 to 3 days after operation by neuroendoscopic therapy and their serum levels were not changed. Conduslon The systematic inflammatory reactions in patients with TSH after neuruendoscopic therapy are mild, but the local inflammatory reactions are significant. The local inflammatory reactions of subdural space and interventional operation are necessary for the absorption and closure of refractory TSH.
