摘  要:目的观察不同剂量舒芬太尼在手术患者全麻诱导期对机体血流动力学的影响。方法将60例择期行脊柱手术患者随机分为S1、S2、S3三组,各组诱导所用舒芬太尼剂量分别为0.2、0.4、0.6μg/kg。各组分别于诱导前(T,基础值),插管前即刻(T0),插管后1 min(T1)、3 min(T2)、5min(T3),记录患者SBP、DBP、MAP、HR、SPO2。结果同组比较:各组不同时点的SBP、DBP、MAP、HR具有统计学意义(P<0.05);各组T0时SBP、DBP、MAP、HR均低于T时(P<0.05)。组间比较:T0时S1、S2组MAP高于S3组(P<0.05);T1、T2时:S1组SBP、DBP、MAP、HR均高于S2、S3组(P<0.05);T3时:S1、S2组SBP、MAP、HR均高于S3组(P<0.05),S1组DBP高于S2、S3组(P<0.05)。结论和0.2μg/kg舒芬太尼相比,0.4μg/kg和0.6μg/kg均能有效抑制插管时应激反应,0.4μg/kg舒芬太尼更有利于患者血流动力学稳定。Objective To observe the effects of ditterent dosage ol Sufentanil in general anesmesia intubation hemodynamics . Methods 60 cases of spinal operation were randomly divided into 3 groups. The systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP) and heart rate (HR) were recorded before intubation (T), in intubation (T0), 1 minute later (T1), 3 minute later(T2 ), 5 minute later (T3), and at the same time, Venous blood was obtain to detect cortisol(Cor) throughradioimmunossay (RIA). Results The values of MAP,DBP, HR were reduced significantly at TO when compared with those at T ( P 〈 0.05 or P 〈 0.01), and the values in Group Three showed greater reduction than Group One and Group Two. In Group One, the values of MAP and HR at T1 were higher than at T, also higher than those in Group Two and Group three when determined at the same time, but returned the original levels at T2 and. T3. In Group three, No significant differences were found in values of MAP and HR at T( PP 〉0.05), but declined from T1 to T3, and became lower at T, to T3 than the original values at T, with 2 cases having the HR of 55 times per minutes. No significant differences were found between the values of MAP and HR at T1 ,T2 and T3 in Group two . The concentration of Cot was found higher than the original value at T1 and T2 in Group One ( P 〈0.05 or P 〈 0.01 ), while no significant differences were found at other time ( P 〉0.05 ). Conclusion Compared with the dosage of 0.2μg/kg, the dosage of 0.4μg/kg and the dosage of 0.6μg/kg can both effectively prohibit the stress responses in intubations, and 0.4μg/kg of sufentail can produce more rheological stability.
