摘　　要：目的探讨DNA结合/分化抑制蛋白-1(Id-1)在病毒性乙型肝炎、乙型肝炎肝硬化和乙肝病毒感染相关肝细胞癌(HCC)中的表达及其临床意义。方法免疫组织化学SP法研究病毒性乙型肝炎40例、乙型肝炎肝硬化30例、HCC65例及癌旁肝组织40例中Id-1的表达情况。结果Id-1在HCC中表达上调,与分化程度呈正相关(r=0.550,P<0.001),在无转移肿瘤中及≤5cm肿瘤中表达上调(P<0.05);Id-1在病毒性乙型肝炎、乙型肝炎肝硬化及HCC中的表达、在病毒性乙型肝炎与HCC及乙型肝炎肝硬化与HCC表达的差别有统计学意义(P<0.05)。结论Id-1在乙型肝炎病毒性肝脏疾病中的表达上调,为原发性HCC的诊断及治疗提供一定的理论依据。Objective To study the expressions of Inhibitor of differentiation/DNA binding-1 （Id-1） in different HBV-related liver disease（type B viral hepatitis, hepatocellular carcinoma with surrounding HBV-related chronic helatitis, and HBV-related liver cirrhosis）, and its potential clinical implication. Methods 65 cases of HCC specimens and 40 cases of adjacent non-tumor liver tissue were obtained from 65 patients with different cell differentiation undergoing hepatectomy for HCC, 40 cases of type B viral hepatitis specimens and 30 cases of HBV-related Liver cirrhosis specimens were abtained by pereutaneous liver biopsy,and 15 cases of normal liver tissue specimens were used for comparison. Immunohistochemical technique was applied to detect the expressions of Id-1. Result The positivity and intensity of Id-1 in chronic viral liver tissues increased from chronic hepatitis, cirrhosis to HCC, and were significantly higher than those in normal liver tissues. Id-1 expression level was positively correlated with the histological grade, also correlated inversely with tumor size and presence of metastasis; and with Nemenyi analysis, expressions of Id-1 in HCC were higher than in type B viral hepatitis and in HBV-related Liver cirrhosis. Conclusion The results of the current study suggested that increased expression of Id-1 may play an important role in the early step of hepatoearcinogenesis, and might serve as a useful marker for determining patients with HCC.
