利用分段栓塞法行支气管动脉栓塞治疗大咯血的临床护理

摘　　要：目的:探讨经普通导管利用分段栓塞法行选择性支气管动脉栓塞治疗大咯血的临床护理及疗效。方法:本组47例患者咯血量>300ml/24h,均采用经普通导管利用分段栓塞法行选择性支气管动脉栓塞;栓塞剂术前制备和消毒,明胶海绵颗粒:1.0mm×1.0mm×1.0mm3、1.5mm×1.5mm×1.5mm3、2.0mm×2.0mm×2.0mm,真丝线段长约1.0mm;术前急救准备,术中密切监护,术后精心护理并随访观察8～24个月。结果:本组47例患者即刻止血43例,咯血明显减少3例,有效率97.9%(46/47),1例止血无效。医、护人员密切配合,手术顺利实施,无严重并发症(脊髓动脉损伤)发生。结论:经普通导管利用分段栓塞法行选择性支气管动脉栓塞治疗大咯血是安全有效的治疗手段,加强围手术期护理,对于提高手术成功率,降低并发症具有重要意义。Objective：To evaluate the efficacy of clinical nursing in fractional bronchial artery embolism to treat haemoptysis. Mothods：47 patients with hemoptysis （hemoptie blood〉300ml/24h）,were treated.The clinical nursing before operalion included psychological nursing,preparation and sterilization of embolus made of silk thread and gelatin in different size,and making the first aid ready.During operation,monitored the patients earefully.After opera tion,looked after the patients intensively and followed up for 8-24months. Results：Hemoptysis stopped at once in 43 eases, decreased greatly in 3 cases, but did not stop in one case.The effective rate was 97.9 percent.Doctors and nurses worked concert edly, the operations were successful with no severe complications such as spinal cord injury. Conclusion：Bronchial artey embolism by fractional embolization to cure haemoptysis is effective and safe ;enhanced perioperative nursing is important to improve suceeeful operation and reduce complications.
