活动性肺结核患者诊断指征与抗体水平观察

摘　　要：目的:调查痰结核菌培养阳性、培养阴性、有空洞和无空洞活动性肺结核患者(TB)血清抗PPD抗体和抗38KDa抗原抗体阳性率和特异度。方法:采用SPA-ABC ELISA检测各组TB和健康人血清PPD抗体和抗38KDa抗体。结果:培阳TB、培阴TB与健康人血清PPD抗体和38KDa蛋白抗体检测阳性率分别为70.6%、36%、3.8%和84.7%、76%、0.96%,38KDa和PPD特异度分别为91%和87.8%;空洞TB、无空洞TB和健康人PPD抗体和38KDa蛋白抗体的阳性率分别为79%、51.2%、4.95%和85.5%、73.2%、1.98%,38KDa抗原和PPD用于空洞结核病血清学诊断的特异度分别为93%和91.9%。结论:38KDa抗原用于血清学诊断对活动性肺结核有很高的阳性率和特异度。Objective：To investigate the antibodies to PPD and to 38KDa protein in serum and the rates of detec- tion in patients with culture positive （85 cases）, culture negative （125 cases）, cavitary lesions （62 cases）, noncavitary lesions （82 cases） an in healthy control （205 cases）. Methods ：Antibodies to PPD and to 38KDa protein in the patients and healthy control were detected by SPA-ABC ELISA. Results：The sensitivities of detection for antibodies to PPD in the patients with culture positive,culture negative and healthy control were 70.6%, 36% and 3.8% ,and the antibody to 38KDa protein were 84.7%, 76% and 0.96% respectively. The sensitivities for detecting the antibodies to PPD in pa- tients with cavitary lesions ,noncavitary lesions and healthy control were 79%, 51.2% and 4.95 %, and the antibodies to 38KDa protein were 85.5 %, 73.2 % and 1.98 % respectively. Conclusions ：The sensitivity and specificity of serodiaqnosis with 38KDa protein is very high in active pulmonary tuberculosis.
