微创DHS治疗老年性A1型及A2型转子间骨折疗效分析

摘  要:目的:探讨应用微创法DHS治疗老年性A1、A2型转子间骨折患者的临床疗效。方法:自2001年4月至2007年10月应用微创DHS治疗股骨转子间骨折28例。骨折采用AO分类法:其中A1型10例,A2型18例。记录手术时间、术中出血量、术后住院时间、临床愈合时间,术后观察有无并发症。结果:手术时间45-70min,平均55min;出血量82-125ml,平均97ml;术后住院时间12-18d,平均14.5d;临床愈合时间10-14周,平均11.4周。无死亡病例,所有患者未出现切口感染、下肢深静脉血栓、应激性溃疡等并发症。28例均获得10~24个月(平均14.2个月)随访。结论:微创DHS内固定手术治疗高龄A1、A2型股骨转子间骨折效果良好,但需掌握好适应证,危重患者及严重骨质疏松患者慎用。Objective: To investigate the curative effect for treatment of A1 and A2 intertrochanteric fractures with minimally invasive DHS in the elderly. Method:28 cases of intertrochanteric fractures in the elderly with DHS treatment from Apr. 2001 to Oct. 2007 were reviewed. By AO classification, 10 type A1, 18 type A2. The patients were treated with closed or indirect reduction and fixation with minimally invasive DHS. Result:There were no major postoperative complications in this study . The mean operation time was 55 minutes (range,45 to 70 minutes) , The mean intra- operative blood loss was 97 milliliters (range ,82 to 125 milliliters),the mean postoperative hospital stay was 14.5 days( range, 12 to 18 days) , and the mean healed clinically was 11.4 weeks( range, 10 to 14 weeks ). At the final radiographic follow - up , no collapse, varus deformity, hardware failure. All patients had a clinical follow - up, the mean follow - up time was 14.2 months (range, 10 to 24 months ). Conclusion:The curative effect for treatment of A1 and A2 intertrochanteric fractures with minimally invasive DHS in the elderly was good, but need to master the indications, crtical patients and seriously osteoporosis patients were given cautiously.
