应用QTVI评价RAS抑制剂对高血压患者左心室功能影响的研究

摘　　要：目的应用定量组织速度成像技术(QTVI)检测高血压患者左心室功能的改变,并评价肾素-血管紧张素系统(RAS)抑制剂对高血压患者左室功能的影响。方法选择高血压患者68例,按是否服用RAS抑制剂再分为RAS抑制组及非RAS抑制组,并设立健康对照组。测量二尖瓣环室间隔侧和左室侧壁侧QTVI曲线上心室收缩期速度峰值(Sa)、心室舒张早期速度峰值(Ea)、心室舒张晚期速度峰值(Aa),并计算左室平均峰值。结果高血压组的Sa波峰值、Ea波峰值及Ea/Aa比值均低于正常对照组(P<0.01);RAS抑制组的Sa波峰值(6.28±1.28)cm/s与正常对照组(6.64±1.63)cm/s比较差异无统计学意义,与非RAS抑制组(5.37±1.14)cm/s比较差异有统计学意义(P<0.01)。结论抑制RAS能阻止或逆转高血压所致的左心室收缩功能损害;QTVI能在常规超声心动图指标异常之前检测到高血压患者的左心室收缩功能受损。Objective To explore the change of left ventricular function in patients with essential hypertension and assess the effects of Renin - Angiotensin System （RAS） inhibitors on left ventrieular by quantitative tissue velocity imaging （ QTVI）. Methods 68 patients with essential hypertension and 37 healthy subjects were randomly selected. Hypertensive patients were divided into two groups according to whether RAS inhibitors were used. Peak velocity of the tissue velocity （Sa, Ea and Aa） was measured at mitral annulus （ septal and lateral）, and mean peak velocity was calculated by averaging data. Results Mean Sa levels, mean Ea levels and Ea/Aa in the hypertensive patients group were significantly lower than that in healthy subjects. Mean Sa levels in hypertensive patients who had been taking RAS inhibitors （6.28 ± 1.28 cm/s） was significantly higher than that in hypertensive patients who had not been taking RAS inhibitors （5.37 ± 1.14 cm/s）. Conclusion RAS inhibitors can stop or reverse the damage on left ventricular in essential hypertensive patients; Quantitative tissue velocity imaging can detect the left ventricular dysfunction in hypertensive patients before manifestation of abnormal conventional echocardiography parameters; QTVI can assess the effects of RAS inhibitors on left ventricular function in patients with essential hypertension.
