摘  要:目的用平均红细胞内血红蛋白含量(CH)及直接测定的单个网织红细胞内血红蛋白含量(CHr)两个参数作联合试验和对照,研究其在诊断缺铁性贫血(IDA)中的应用价值。方法对本院272例标本进行CH和CHr的测定。其中经骨髓检查或血清铁蛋白确诊的IDA患者116例(包括经治疗的IDA患者40例,初诊的IDA病人76例),健康对照者156例。标本采用ADVIA2120全自动血细胞分析仪进行检测。用ROC曲线进行评价,选取合适的临界点,计算和比较相关的敏感性和特异性。结果IDA患者的CH和CHr值均比正常对照组低(P<0.01)。经ROC曲线分析CH和CHr在诊断IDA时的曲线下面积均为0.98,在临界值处CH的灵敏度为97.3%,特异性为96.4%,CHr的敏感度为94.7%,特异性为96.4%。而两者联合诊断,则其灵敏度为93.4%,特异性为98.2%。结论CH和CHr可作为缺铁性贫血的诊断指标,尤其适用于IDA的早期诊断及疗效的监测。Objective To study the application of red cell hemoglobin(CH)and cellular hemoglobin reticulocyte(CHr)in diagnosing iron deficiency anaemia(IDA).Methods The CH and CHr levles of 116 IDA patients(40 cured patients and 76 outpatients)and 156 healthy subjects were tested by ADVIA 2120 automatic blood cell analyzer.The IDA patients were clinically diagnosed according to domestic IDA diagnostic criteria.Receiver operating characteristics(ROC)curve analyses were used to compare the performance of CH and CHr.Sensitivity and specificity at the critical point were calculated and compared.Results The sensitivity of CH,CHr and their combination in diagnosing IDA were 97.3%,94.7% and 93.4% respectively.Their specificity were 96.4%,96.4% and 98.2% respectively.The CH and CHr levels of IDA patients apparently decreased(P〈0.01)compared with those of healthy subjects.Conclusion CH and CHr can be applied as diagnostic indexes for IDA,especially for the early-stage diagnosis and therapy effect monitoring.
