摘  要:目的用磁共振波谱(MRS)观察特发性震颤(ET)患者小脑皮质代谢物变化情况。方法ET患者11例为病例组,健康人7名为对照组,对双侧小脑皮质进行磁共振波谱(MRS)检查,计算乙酰天门冬氨酸(NAA)、肌酸(Cr)、胆碱(Cho)比值,并对每例患者进行临床评分(Fahn-Tolosa-Marin)。结果病例组同侧小脑皮质NAA/Cr较对照组减低(0.78±0.05,0.90±0.16;P=0.03)。病例组双侧小脑皮质代谢物比值与症状评分间无明显相关性。结论ET患者小脑皮质存在神经元的损伤或丢失,提示ET很可能是一种神经退行性疾病。Objective To investigate the metabolic alterations in the cerebellar cortex of patients with essential tremor (ET) using magnetic resonance spectroscopy (MRS). Methods Eleven ET patients and 7 healthy controls underwent ^1H-MRS examination to measure levels of intraeellular metabolites, including ratio of N-acetyll-aspartate (NAA) and total choline (Cho) to total creatine (Cr) in cerebellar cortex with a 3.0T Philips Achieva System. Results The mean (standard deviation) ipsilateral cerebellar cortical NAA/Cr decreased in ET patients compared with controls (0.78±0.05 vs 0.90±0.16, P=0.03). There was no relationship between eerebellar cortical NAA/Cr and dominant arm tremor severity (P〉0.05). Conclusion The reduction in cerebellar cortical NAA/Cr indicates that there is neuronal damage or loss in ET, suggesting that ET may be a neurodegenerative disease.
