睾丸热压内裤抗生育效果的初步实验:附11例报告

摘　　要：目的观察睾丸局部热压对成年男性精子及性激素分泌的影响。方法11例已育成年健康男性志愿者，穿43℃睾丸热压内裤（THSU），每周连续2d穿2次，每次穿50min，3个月为1个实验周期。在实验前、实验中和实验后分别取精液和血液，常规方法分析精子功能参数、低渗肿胀实验（HOST）和精子染色质扩散（SCD）实验；化学发光免疫分析法测定血清皋酮（T）、卵泡刺激素（FSH）和黄体生成素（LH）含量。THSU实验前的相关指标作为自身正常对照，停止实验后连续观察3个月（11例）和6个月（9例）。结果THSU实验前精子密度、活动率（a＋b＋c级）、畸形精子、HOST和SCD精子DNA完整比率分别平均为（85.8±19．7）×10^6／ml、（46．7±17．9）％、（35．7±7．9）％、（65．1±11．5）％和（86．7±10．3）％，THSU实验2个月后，上述各项指标明显改变，3个月后分别平均为（8．7±6．6）×10^6／ml、（18．2±7．2）％、（53．6±5．2）％、（15．6±8．2）％和（26．2±6．7）％，与实验前比较均有显著性差异（P〈0．01）；停止THSU实验2、3和6个月后，上述各项指标又恢复到实验前水平。血清T、FSH、LH实验前、中、后均无明显改变。结论THSU作为男性避孕方法具有一定前景。Objective.To observe the effect of testis heat stress on male fertility by measuring the changes in semen sperm count, sperm motility and DNA damages as well as in serum hormones. Methods：Eleven healthy men with proven fertility were chosen for the trial. The volunteers were asked to wear specially designed testis heat stress underpants （THSU） with a constant 43℃ apparatus, two days a week, and 50 minutes per day, for a period of continuous 3 months. Routine semen analysis and hypo-osmotic swelling test （HOST） as well as sperm chromatin dispersion （SCD） examination were carried out before, during and after the application of TI-ISU. Serum follicle stimulating hormone （FSH）, luteinizing hormone （LH） and testosterone （T） were measured by chemiluminescent immunoassay. Results：The mean parameters of sperm density, motility, abnormality, the HOST scores and sperm DNA integrity as measured by SCD test before THSU were （85.8±19.7） ×10^6/ml, （46.7±17.9）%, （35.7±7.9）%, （65.1±11.5）% and （86.7±10.3）% respectively. 2 months after THSU the parameters were significantly altered. By 3 months, （53.6±5.2） （P〈0.01）. After two, three and six mon a dramatic decrease or increase in above parameters [-（8.7±6.6） %, （15.6 ±8. 2）% and （26.2 ± 6.7）%, respectively] was observed ths of recovery （termination of the use）, all parameters gradually returned to the pre-trial levels. The THSU did not alter serum concentrations of T, FSH and LH. Conclusion.THSU could be an excellent and acceptable way of contraception for males.
