摘　　要：目的应用胫后动脉内踝上皮支逆行岛状皮瓣修复足踝部软组织缺损的临床效果。方法自2006年8月至2008年10月,应用胫后动脉内踝上皮支逆行岛状皮瓣转移修复各种原因所致的足踝部软组织缺损10例。结果9例术后皮瓣全部成活,1例皮瓣远端少许坏死,皮瓣面积为4cm×6cm～10cm×13cm随访6个月～2年,效果满意。结论应用胫后动脉内踝上皮支为蒂,形成逆行岛状皮瓣,可以不牺牲小腿主要血管胫后动脉即可修复足踝部大部分皮肤软组织缺损,手术方法简单、安全,是一种比较理想的修复足踝部软组织缺损的方法。Objective To explore therapeutic effects of grafting superior regressive island skin flap supplied with the medial malleolus cutaneous branch of the posterior tibial artery in management of ankle soft tis- sue defects. Methods Sine 2006-8 to 2008-10,10 cases of ankle soft tissue defects caused by various kinds of reasons were treated with superior regressive island skin flap supplied with the medial malleolus cutaneous branch of the posterior tibial artery grafting. Results 9 cases＇ flaps were healed primarily; 1 case, distal part of the flap were partially necrotic. The area of flaps ranged from 4 cm× 6 cm to 10cm × 13 era. All cased were followed up from 6 to 24 months and achieved satisfactory results. Conclusion Grafting of superior regressive island skin flap supplied with the medial malleolus cutaneous branch of the posterior tibial artery as pedicle can repair subtotal ankle soft tissue defects without damaging surae main artery-posterior tibial artery. It is an easy safe and satisfactory operating method to repair ankle soft tissue defects.
