超声组织速度成像评价重度子痫前期患者左心室收缩功能的价值

摘　　要：目的探讨超声组织速度成像(TVI)评价孕妇左心室功能的价值。方法采用TVI技术及常规、M型超声检查20例重度子痫前期孕妇(观察组)和21正常孕妇(对照组)左心室功能指标。结果观察组二尖瓣口血流舒张早期峰值流速及其与舒张晚期峰值流速比值,左心室前壁基底段和中段心肌的收缩期峰值速度、舒张早期峰值速度、EF均显著低于对照组(P均<0.05);而二尖瓣舒张晚期充盈峰、肺静脉心房收缩期反向血流充盈峰、相对室壁厚度均显著高于对照组(P均<0.05);胎儿左心室侧壁中段心肌的速度峰值均减小,但无统计学意义。结论重度子痫前期即存在左心室心肌肥厚、舒缩功能减低等结构功能异常,但尚未引起胎儿左心室壁功能异常;TVI可用于评价重度子痫前期孕妇心功能及胎儿室壁功能。Objective To investigate the Value of quantitative tissue velocity imaging （QTVI） in the assessment of left ventrieular function in patients with severe preeclampsia. Methods The index of left ventrieular function in 20 patients with severe preeclampsia（ observed group） and 21 healthy controls （control group）were measured by QTVI , conventional and M-type Doppler imaging. Results E, E/A,BaS, Vs, Ve and LVEF in the observed group were significantly higher than those in the control group. While A, AR were significantly lower than those in the control group. Conclusions Myocardial hypertrophy and reduced diastolic function of left ventricle can be found in patients with severe preeclampsia. QTVI offers a method for evaluating the heart function of patients with severe preeclampsis and ventricular wall function of fetus.
