摘  要:目的探讨195例国人正常左肾静脉(LRV)的多层螺旋CT(MSCT)表现并测量有关数据,确定LRV各项测量指标95%可信区间,为LRV病变的影像学诊断提供依据。资料与方法回顾性随机选取195例正常肾血管的多层螺旋CT血管成像(MSCTA)或腹部MSCT增强扫描资料,按年龄分为A、B、C三组,按性别分为男、女两组。利用MSCT血管后处理技术,观察LRV及其属支的空间走行、与邻近结构关系和断面形态,测量肠系膜上动脉(SMA)与腹主动脉(AA)之间夹角(a)、SMA与AA三角间隙内LRV断面中点水平SMA后缘与AA前缘距离(d);在LRV断面上分别测量:SMA与AA间隙内LRV前后径(r1)和截面积(s1);近肾侧LRV最大断面处前后径(r2)和截面积(s2),以及两者的比值qr(r2/r1)、qs(s2/s1);测量LRV长度(l)。进行统计学分析。结果a、d、r1、r2、s1、s2、qr、qs、l值分别为:(49.45±21.11)°、(13.0±6.3)mm、(6.0±2.5)mm、(10.7±2.6)mm、(83.0±37.1)mm2、(127.2±43.4)mm2、(2.14±1.19)、(1.73±0.82)、(75.9±12.0)mm。各值呈正态分布,其95%可信区间分别为:(49.45±2.96)°、(13.0±0.9)mm、(6.0±0.4)mm、(10.7±0.4)mm、(83.0±5.3)mm2、(127.2±6.3)mm2、(2.14±0.17)、(1.73±0.12)、(75.9±1.7)mm。性别组间仅qs差异无统计学意义(P>0.05),其余各值差异有统计学意义(P<0.05);年龄分组间仅LRV长度(l)一项差异有统计学意义(P<0.05),其余各项差异无统计学意义(P>0.05)。结论MSCT能够直观观察LRV的形态及与邻近结构关系,并进行相关测量,有助于LRV相关疾病的诊断。Objective To investigate the appearance and relative data measured of the normal left renal vein (LRV) using multi-slice spiral computed tomograpby (MSCT) , and to ascertain the 95% confidence interval(CI) so as to provide the accordance in diagnosing the disease of LRV. Materials and Methods MSCT angiography (MSCTA) data of 195 normal kidneys and renal vessels were retrospectively reviewed. All LRVs were respectively divided into 3 groups according to age and 2 groups according to sex. The anatomy, course and relationship to the adjacent structure of left renal vein (LRV) ,and its branches were observed. The angle (a) between SMA and the abdominal aorta(AA) , the distance(d) between SMA and AA at the level of LRV passing through, the diameter and the cross areas of LRV through( r1, s1 ) , and at the largest diameter near the renal hilar (r2, s2) were measured and ratio (qr,qs) of r2 to rl and s2 to sl were calculated,the length of LRVs were measured. Results The value of a, d, r1, r2, s1, s2, qr(r2/r1 ) , qs(s2/s1 ) and 1 were (49.45±21.11)°, (13.0±6.3) mm, (6.0±2.5) mm, (10.7±2.6) mm , (83.0±37.1) mm^2, (127.2±43.4) mm2, (2.14±1.19), (1.73±0.82), (75.9±12.0) mmo The 95% confidence interval of a, d, r1, r2, s1, s2, qr (r2/r1) , qs(s2/s1) and 1 were(49.45 ±2.96)°, (13.0 ±0.9) mm, (6.0±0.4) mm, (10.7±0.4) mm, (83.0 ± 5.3) mm^2, ( 127.2 ±6.3) mm^2, (2.14 ±0.17), ( 1.73 ±0.12), (75.9 ±1.7) mm. There were statistically significant between 2 sex groups except qs, and there were no statistically significant between 3 age groups except 1. Conclusion MSCT is an optimal assessment tool to demonstrate the profile,size and relationship to the adjacent structure of the LRV, which is helpful in diagnosing the disease of LRV.
