摘 要:目的比较相同碘浓度和注射流率、不同对比剂注射方案情况下,主动脉多层螺旋CT血管成像(MSC-TA)的应用效果。资料与方法将90例可疑主动脉病变患者随机分为A、B、C3组(每组30例),在对比剂浓度300mgI/ml、注射流率3ml/s相同的情况下,分别采用不同的注射方案行主动脉MSCTA。A组:注射对比剂100ml;B组:注射对比剂60ml后用40ml生理盐水冲刷;C组:注射对比剂85ml。扫描延迟时间采用自动触发技术。测量升主动脉、T7及L2水平降主动脉、主动脉分叉处CT值;由两位影像诊断高级职称医师对血管强化程度、重组血管清晰度进行评价。结果3种注射方案在升主动脉、T7及L2水平降主动脉、主动脉分叉处的平均强化程度分别为255HU、240HU、247HU,3组间差异无统计学意义,3组重组血管清晰度差异无统计学意义,93.33%(84/90)清楚显示两侧肾动脉及分支,6.67%(6/90)显示两侧肾动脉及分支欠清楚。结论应用对比剂60ml注射+40ml生理盐水冲刷行主动脉MSCTA能够满足主动脉检查要求,减少了对比剂的用量。Objective To study the effects of different dose of contrast material with the same concentration and injection rate on MSCT angiography. Materials and Methods 90 patients suspected aortic-diseases were divided into three groups randomly and received MSCT aortic-angiography with three different injection protocols respectively; group A: 300mg/ml × 3 ml/s, 100 ml contrast material, group B:300mg/ ml × 3 ml/s ,60 ml of contrast material was injected followed by flushing with 40 ml saline solution, group C : 300mg/ ml× 3 ml/s, 85 ml contrast material. The scanning delay time adopt the auto-trigger technique and the CT values of the ascending aorta(AA) ,T7 and L2 level of descending artery, aortic artery fork were respectively measured. The enhancement of arteries and the definition of the reconstructed vessels were evaluated respectively by two experienced radiologist. Results The mean degree of enhancement in the ascending aorta(AA) ,T7 and L2 level of descending artery,aortic artery fork were similar (about 255 HU,240 HU,247 HU) in group A, group B and group C, which had no significant difference and also in the quality of reconstruction images between group A and group B,group C, 93.33% (84/90) of two kidney artery and its branches clearly were clearly depicted,6.67% (6/ 90 ) were not clearly depicted. Conclusion The injection protocols with 60 ml + 40 ml saline is competent for MSCT aortic-angiography, which can reduce the dose of contrast material.
