摘  要:目的探讨后腹腔镜治疗肾囊肿时结合使用硬化剂,以减少肾囊肿复发率。方法不同硬化剂体外处理肾囊肿壁,观察不同时间段囊肿壁硬化效果,选择适当硬化剂。86例单纯性肾囊肿,其中单纯采用后腹腔镜去顶术38例,采用后腹腔镜去顶术并术中结合硬化剂治疗48例。平均随访时间22个月(6~36个月),B超复查囊肿大小。结果单纯采用后腹腔镜去顶术组平均手术时间(36.6±15.3)min,住院时间(5.3±1.6)d,长期随访复发4例(复发率10.5%);采用后腹腔镜术中结合硬化剂组平均手术时间(38.2±13.8)min,住院时间(5.6±1.4)d,长期随访无复发。结论肾囊肿后腹腔镜去顶术中结合使用硬化剂,除具有创伤小、出血少和康复快等优点外,能有效降低远期复发率。Objective To explore retroperitoneal laparoscopy with sclerosing agent in treatment of renal cyst for decreasing recurrent rate. Methods Several sclerosing agents were applied to deal with the wall of renal cyst in different periods out of body,for selecting appropriate sclerosing agent. Of 86 patients with simple renal cyst,38 cases were treated by retroperitoneal laparoseopic renal cyst un-roofing alone,and 48 by retroperitoneal laparoscopic renal cyst un-roofing in combination with sclerotherapy. The mean follow-up period was 22 months(ranging from 6 to 36 months). Six, 12,24 and 36 months after treatment, the changes of cysts were re-examined by ultrasound. Results In the retroperitoneal laparoscopic group, the mean operative time was (36.5 ± 15.3) min, the mean hospital stay was (5.3±1.6) days, and 4 cases(10.5 % ) were recurred. In the combined treatment group, the mean operative time was (38.2 ± 13.8) min, the mean hospital stay was (5.6 ±1.4) days, and no recurrence was found during a long-term follow-up period. Conclusion The retroperitoneal laparoscopic renal cyst un-roofing with sclerotherapy has the advantages of lowering long-term recurrent rate efficiently,besides less trauma,less blood loss, and rapid recovery.
